The licensor observed an unsupervised child in the classroom outside the office.
The kindergarten and 3rd grade classroom attendance log did not align with the number of children present.

Licensor observed 2 expired gallons of milk and 2 expired individual boxes of apple jacks cereal.
A piece of border trim surrounding the outdoor structure is broken; bait traps are accessible to children in the outdoor space.
Licensor observed peeling paint on the outdoor concrete stairs leading to the outdoor licensed space and the building foundation.
