There is a(REDACTED) infant in care. The licensor observed that there was an extra soft mattress placed in the pack and play for the infant to sleep on. The educator removed it today before placing the infant down for nap
During the visit today the licensor observed a toddler climbing out of the high chair. The educator and the assistant did not notice until the licensor pointed it out

When the licensors arrived they observed a toddler strapped into an infant chair. The Educator stated that she put her there because she was going to do paperwork. She was the only Educator in the program at the time
A random review of some of the childrens files show that: one child was missing proof of a lead test and 1 child needs a medical on file
A review of the files show that progress reports need updated
The children did not wash their hands before eating munchkins
The gate at the bottom of the stairs leading upstairs was open

When the licensor arrived she observed a toddler to be strapped into an infant chair. The Educator stated she places her there for supervision purposes because the child climbs on everything

The Educator was using an infant seat for a toddler, this is not an appropriate use of equipment because the toddler is not the appropriate age, size for this equipment

Progress Reports
The first aid kit was missing the children’s emergency cards, scissors and a thermometer. The educator added the thermometer during the visit
