Random review of children’s records was completed. General permission does not indicate where children are taken when they go off premises. One file did not have lead screening results. One file had an allergy action plan that was not signed by the doctor.
There were two containers outside that had standing water.

There was a mop bucket containing water that was accessible in the kitchen. There was access to beauty products on top of playshelf. (Cologne and deodorant) Access to a plastic bag was observed in the kitchen from opened water bottle package.

In the second playroom there was a shelf that was not secured. It was observed there were electrical cords accessible and not secured by the television stand.

Assistant did not know where the children’s progress reports were, these could not be reviewed.
There was no gate installed to prevent access to the stairway leading to first floor.
It was observed there were sticky mouse traps accessible in the living room. Discussed safe ways for pest elimination.
Educator has requested for her first floor living room be approved. It was observed that the corners of fireplace hearth has sharp edges. The three baseboard heaters in the living room did not have a covering, leaving access for children to be able to place their hands inside. There is access to the kitchen, it was observed there were sharp knives, scissors, and plastic bags accesible within the kitchen drawers and cabinet. There was access to a portable fan in the family room leading to second exit on first floor level. It the bathroom cabinet and drawers there was access to beauty products.

Emergency contact numbers list did not include educator’s information and did not include emergency number 911.
Outdoor space was observed, there were some hazards that were accessible to children. Some of the hazards observed were a screwdriver and empty alcoholic beverage cans.
It was observed that both pack and plays were showing some wear and tear and must be replaced.
Educator just had a new fence installed and requested that her yard be approved for use. It was observed that there were some gardening tools accessible(rakes and shovels were observed), there was wooden slats across the yard and on the side of the home, a water hose was observed to be laid out across one side of the yard, there was pieces of leftover fencing material and tubing laid across the side of the yard, there was a bucket that appeared to have, rocks and other debris, and a paint spray can was also accessible. The doors to the gate on one side were observed to have a wide opening even when closed. These observations were discussed with the educator during a phone call. Once corrections are completed the educator will submit pictures and there may be a follow up visit. La educadora acaba de instalar una cerca nueva y solicitó que se apruebe el uso de su yarda.
