When the licensor arrived a (REDACTED) was sleeping in a pack and play, there was a blanket on the infant that covered her up to her chest. The Educator walked down the stairs in front of the licensor and took the blanket off the infant as she walked by. The licensor spoke to the Educator about the blanket and she stated she had it on the child’s legs as it was cold in the space. This is the Educator’s 3rd citation for safe sleep.

When the licensor arrived for the visit, the children were sitting at the table eating lunch. The Educator had the news on the television. The content was not appropriate for the children in care. The Educator turned it off at the licensors request.

A review of the children’s records show that 4 children are missing how to reach a parent a work, 1 child was missing his name in the permission to treat with CPR, 1 child had a parents name instead of the child’s name on the CPR authorization, 1 child needs an annual update 1 child did not have the educators name on the permission forms, it said N/A and 1 child has an old EEC enrollment packet that is missing information such as pg. 2 of developmental history and physical description/photo
The confidential allergy list is not updated, it contains previous children with (REDACTED) and 1 current child with allergies. There are 3 children in care currently with (REDACTED). All 3 have current IHCP on file and with the required postings

A review of the children’s records show that one child needed proof a current medical documentation (REDACTED) Some records also had General Permission sections that were not specific to locations off the premises.
There have been 9 times in the last 2 months that the attendance was not accurately documented.
The required emergency postings did not contain all required EEC information: missing, Educator's name/address and an emergency back up person
Parent Handbook
There are 3 children in care that have documented (REDACTED). One of the children did not have that medication at the child care and 1 child had (REDACTED) at the child care. The educator stated that 1 child had the (REDACTED) at the child care but she had misplaced it and could not find it.
While the licensor was reviewing the first aid kit she found an (REDACTED) in the first aid kit. The (REDACTED) was loose in the kit, it was not in the original container with the (REDACTED) label on it. There was no way to know who the (REDACTED) for, if it was current and what it was for.
There was no gate at the bottom of the stairs when the licensor arrived for the visit. There were children under 3 present
The first aid kit did not contain the emergency contact cards for the children in care.
