It was observed that there was a climbing structure (slide) setup without the required 6 foot fall zones from all sides and the required ground surfacing. Outdoor playground handbook will be sent to educator. This structure cannot be accesible to children. There was also a trampoline accessible; trampolines are not approved for use and access must be prevented.

It was observed that educator did not document a progress report every 3 months for children under 15 months old.
There was access to multiple hazards from both the first floor exit and the basement exit. It was observed that there was access to outdoor work materials, some items observed were piles of wooden boards some had nails protruding, there were extension cords, garden tools, ladders, and piles with random items.

It was observed that the diapering table in the new nap room was easily moveable and needs to be secured. There were some low hanging decorations in the new nap room that need to be made inaccessible to the children.

It was observed that the nap room, the educator is requesting to have approved was too dark. Educator must have sufficient lighting in the room to be able to appropriately supervise the children that will be napping in the room.

There was a portable heater in the new nap room. Reviewed with the educator that portable heaters are not approved for use. Educator indicated that she placed portable heater in the room because she felt it wasn’t warm enough. Reviewed with the educator that door to new room should always be open and this will allow for better ventilation.
There were three outlets in the new nap room that were not covered.
Educator needs to add a carbon monoxide detector near the new nap room.
There was one child sleeping in the room that has not yet been approved for use. The purpose of this visit was to make observation of room for approval. It was reviewed with the educator that she cannot utilize the room until it has been approved and added to her licensed space.
