Outdoor space was observed. It was observed that toy storage unit was not sturdy. Educator was previously cited for this. Educator took out toys from the storage to attempt to connect the panels, however, it is still not sturdy. It was observed that the gate installed on the side of the shed had fallen, leaving access to hazards behind the shed. Educator was previously cited for this. Educator attempted to secure the gate during visit, however, one side of the gate could not be secured due to obstruction by large wooden panels. This gate must be up and sturdy at all times to prevent access to hazards.
One file did not have the annual update completed.

Temperature could not be read at time of visit because mini fridge was not plugged in. Educator plugged in during visit.
This investigation inspection recorded no violations or advisories.
The first aid kit did not contain a CPR mask as required. Two children were enrolled in the program, and their emergency cards were not in the first aid kit.
Transportation Safety
On the day of the visit, the educator transported two children on a personal errand and to the park in her vehicle without written consent from the parent. Please submit your program's transportation plan for review.

The licensor observed 1 electrical outlet uncovered by the children’s table. The Educator covered it while the licensor was present
The educator would like to have her backyard approved, the following hazards were observed: there was a smoker that was only partially covered, a wooden stool, the hose was accessible, the fire pit has edges that are a hazard, there are old fence pieces accessible, a post digger 2 things of standing water and balloon pieces.
A review of the children’s records show that 1 child needs an annual update and and updated medical and 1 child did not have a medical on file

During the visit today, the licensors noticed that a plastic bucket that a child was playing with was broken on the bottom and sharp edges were accessible to the child.
The licensor did not observe enough developmentally appropriate toys for the older children in care
Toys/materials
The licensors observed an interaction between an infant and a preschool child, where the infant attempted to bite a preschool child. The preschool child gently pushed the infant away and the Educator did not respond appropriately: she stated “(REDACTED) he is just a baby” to the preschool age child.
The licensor observed that there was an open perfume bottle on the bathroom sink. The licensor also observed that a bean bag had broken open and there were little bean bag pellets on the floor in licensed space
Indoor Hazards
As the licensors were walking up the driveway into the home, they smelled marijuana coming from one of the cars in the driveway. As they were walking by the car they observed a household member/approved assistant sitting in the passenger's seat of the car and loose marijuana was visible on the “marijuana tray”in the middle console. Regulations prohibit smoking on the premises of the child care home when children are in care.

The licensor observed the smoke detector at the bottom of the stairway from the first floor to the basement was taken down. Additionally the smoke detector on the first floor outside the sleeping area was taken down. There was 1 working smoke detector in the basement space
The trash in the bathroom was not covered today
Although the infant was awake today, the licensor viewed the pack and play where he sleeps. The licensor observed an additional pack and play mattress in the crib. This is the Educator’s second safe sleep citation
This follow-up visit inspection recorded no violations or advisories.

The licensor observed the window cord string in the kitchen to be longer than 6 inches and accessible to the children
The licensor observed the liquid soap to only have a tiny amount in the bottom of the container. There was bar soap on the sink. The Educator stated that she was using the bar soap because the liquid soap was empty. The licensor informed her she must use liquid soap so she used the small amount that was left in the bottle. The children did not dry their hands after washing them. There were no paper towels or individual way for the children to dry their hands accessible to them

The records for all five enrolled children were reviewed. The Educator has (REDACTED) enrolled child. The enrollment packet for this child did not have all of the EEC required information. Discussed using the EEC enrollment packet, found on the EEC website, so that all required information is documented. Another child needed an Annual Update form.
The first aid kit was missing an instant cold pack and the children’s emergency cards.
In the bathroom there were 2 drawers with hazards accessible to children (scissors, topical ointment and diaper cream). There were also a drawer and a cabinet in the kitchen area with hazards Cato children (plastic bags and long cords, which are considered suffocation and strangulation hazards.)
the educator would like to license her backyard she will need to correct the following to be able to have her backyard licensed. During today’s visit the licensor observed a wood pile near the back fence that would be accessible to children, a wheelbarrow that would be accessible to children, a child’s pool that was filled with rocks and water. There were also 4 taxis parked in the backyard playspace. There was no gate/fence preventing the children from accessing the road on 1 side of the yard

Progress Reports
The gate on the fence was broken, which did not allow the gate to close and be secure. There was a grill accessible that was not covered. There was a mop bucket in the hallway when the licensors arrived, the educator put it away during the visit. There is an extension cord accessible in the living room, a kitchen drawer had a screwdriver and plastic bags accessible, the door to the play fridge was broken.

The licensor asked the Educator to do an evacuation drill with the children in care. The Educator pushed the smoke alarm, then opened the door for the 4 older children to run outside, while she picked up the 2 infants in care. The children ran into the backyard and just ran around the backyard space. They did not know where the meeting spot was
The licensor observed the pack and play the infant sleeps in to have a soft, fuzzy blanket on top of the sheet, and another blanket underneath the sheet. The Educator stated that she thought it was better for the infant's body to sleep on a soft surface. The licensor had her remove the extra blankets and just have the tight fitted sheet when she put the infant down for a nap. This is the first safe sleep non compliance
The licensor observed the Educator to place an (REDACTED) in an infant seat and hand him his bottle to drink. Infants must be held while bottle fed
