Written Parental Consent
A review of 5 children records, Licensor noted the following: 1. One child had a first aid medical consent form dated 11/20/23 transportation and off site permission forms dated 11/13/23.
Two children were missing the parental consent form for the medications,

One child on an emergency medication was missing the IHCP form.
One child on an emergency medication did not have the consent form.
Accurate attendance; arrival/departure times
At the time of the visit the attendance roster found several children arrival and departure times were not documented on the following days: 7/31, 8/8, 8/22, 8/24.
At the time of the visit there were 6 children on the attendance but there were 8 children present. 2. At the time of the visit the previous attendance sheets for that week or prior were not available for review.
The refrigerator did not have a thermometer inside.

A review of two staff records found that: one staff record was missing the date of interview, resume, verification of references, staff form, and staff orientation form, evidence of a physical examination and MMR immunization records.

1. At the time of the visit the daily attendance was not available for review. 2. The attendance reviewed was for March and it was incomplete with the dates 3/3/-3/9 and the arrival and departure times of all children were not documented. 3. The months of April and May were not available for review.
The emergency evacuation log did not include the exit routes used.
This follow-up visit inspection recorded no violations or advisories.
