The name, DOB, and Chronic Health Condition were the only items documented on the plan. The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered.

A review of three educator records indicated the following: -Two had incomplete Staff Information Forms. -One did not have documentation of completing the EEC Essentials Training. -One did not have documentation of a current physical examination. -One did not have evidence of being immunized in accordance with the recommendations of the Department of Public Health. -One did not have evidence of current first aid training.
One educator had a Background Record Check (BRC) result in their file. The hiring authority shall maintain EEC's approval of the fingerprint-based check in a locked, secure file separate form the candidates personnel file.
The licensee did not maintain as part of a child’s record, an individual health care plan for a child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The same child had an expired medication. (REDACTED)

The Licensee did not have documented observations of educators. Supervision must include, but not be limited to observation of educators while working with children at least every two months by staff with a lead teacher, site coordinator or higher qualifications consultation with educators regarding children’s individual needs and communication with families; and documentation of all observations and consultations.
The licensee provided graham crackers and water for snack. The licensee must design and implement a nutrition program that meets the U.S.D.A. guidelines for the nutritional and dietary needs and feeding requirements of each child, including those of children with disabilities.
