A review of four staff records indicated the following: -One did not have evidence of an annual physical examination. -One did not have evidence the educator has been immunized in accordance with the recommendations of the Department of Public Health. -Two did not have a current PQ Registry. All educators must register annually with the Department of Early Education and Care in accordance with EEC policies and procedures.

Every program that is not located in a residence and every program that has a licensed capacity of more than ten children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the applicable 780 CMR: The Sate Building Code.
