-The program did not have evidence of completing 2 hour monthly staff meetings for the following months: February 2026, September 2025-February 2025. -The program only documented one hour monthly staff meetings. The program is required to hold two hour staff meetings monthly.
A review of children's records indicated the following: -One did not have an Individual Health Care Plan for a known medical condition. All children must have a current IHCP and medication consent form for known medical conditions.
A review of staff records indicated the following: -There was minimal evidence that the program has been completing bi-monthly observations on educators. -Two educators did not have documentation of annual assessments for multiple years. Educators must have written assessments annually. -One educator had an incomplete verification of experience form. -One educator did not have evidence of a current physical examination.
The first aid kit did not have a CPR mouthguard as required.
One child did not have a current medication consent form to administer emergency medication to the child for a known allergy.

A review of Staff Records indicated the following: -The program did not have documentation of all educators having bi monthly observations by either a site coordinator or higher on a bi monthly basis. -The program administrator did not have documentation of completing yearly evaluations of every staff member caring for children.

This investigation inspection recorded no violations or advisories.
A review of educator records indicated the following: -The staff do not have documentation of bi monthly observations by the supervisor. -The staff did not have current documentation of staff evaluations as required. -The staff did not have documentation of completing professional development trainings as required.

A review of fire drills at the program indicated the following: -The 10/2/23 fire drill did not have documentation of the amount of time it took to evacuate all the children from the program. -There was no documentation of a fire drill being completed in June of 2023.
A review of staff records indicated the following: -Four did not have documentation of having a complete physical exam prior to working with children. -One did not have documentation of having immunity to Mumps, Measles, and Rubella. -Four did not have documentation of completing a certified First Aid Course in the first three months of working with children. -None of the educators assigned to supervise the children had documentation of completing a First Aid and CPR Course. -One did not have documentation of an interview.

The licensee did not have documentation of completing evacuation drills for June 2021, September 2021, November 2021, and January 2022. Additionally, the evacuation drill for December 2021 did not have documented number of children present and the day it occurred.

A review of the snack menu indicated the following: The licensee provided fruit punch and rice cakes for snack. The licensee must design and implement a nutrition program that meets the U.S.D.A. guidelines for the nutritional and dietary needs and feeding requirements of each child, including those of children with disabilities.

A review of four staff records indicated the following: -One did not have documentation of an interview. -One did not have a completed staff information form. -Three did not register annually with the Department of Early Education and Care. -Two did not have documentation of a current physical examination at the time of each license renewal or more often if, in the Department’s or licensee’s judgment, a medical condition requires a greater frequency. -Two did not have evidence that the educator has been immunized in accordance with the recommendations of the Department of Public Health. -One did not have within six months of employment, and must maintain thereafter, current certification of training in basic first aid appropriate to the population served.

The licensee did not have documentation of completing an evacuation drill in January 2023.

The written parental consent and the licensed health care practitioner authorization shall be valid for one year,
-One injury/incident report had the child's first name, date of injury and type of injury. The rest of the information was not completed. The licensee must maintain a record of any unusual or serious incidents including but not limited to behavioral incidents, injuries, property destruction or emergencies. These reports must be reviewed by the licensee or Program Administrator on a monthly basis.
-One did not have evidence of a completed background record check.
In the cafeteria the trash was not covered.
In the cafeteria, on the stage, there were approximately 30 large boxes stacked on top of each other.
The license did not have a current written schedule listing all persons on duty, including volunteers. The schedule must show compliance with at least the minimum educator ratios at all times, including during breaks and planning time. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.
