Attendance
There are two dogs in the home, on of which was in the child care space at the time of the visit. There were no vaccinations or proof of license available for either dog.
There were no professional development hours for the Educator available for review at the time of the visit.
There was no staff record available for review during the visit. This includes a current medial for the assistant and the assistants training hours.
The attendance records for the day of the visit were not completed. The certified assistant completed it while the licensor was present.

The licensor observed that a (REDACTED) was given a bottle in the crib. The child fed herself the bottle as she laid down for nap. The educator removed the bottle at the licensors request.

When the licensor arrived there was not an individual way for the children to dry their hands. The Educator brought paper towels into the bathroom for the children to use after handwashing
The Educator has two children in care who require life saving medication and she did not have the confidential allergy list posted.

The Educator did not have an individual way for the children to dry their hands, the children were all using one hand towel to dry their hands.
The Educator did not have enough appropriate napping materials for the children enrolled. The Educator has 2 infants enrolled (11m) and only one crib in the childcare space. Both infants were present on the day of the visit. The Educator has been napping one of the infants on a cot on the floor. The Educator got a pack and play out of storage and set it up during the visit at the request of the licensor.
The Educator has prescription medication for a child that does not have a prescription label attached to it as required.

A review of 6 children’s records show that: 1 child had an incomplete enrollment packet it was missing pgs. 2,4&6. Additionally 1 child needs an updated medical and 1 child does not have a medical or lead test on file
The last documented fire drill was in May of this year.

The licensor observed a plastic slide measuring over 18 inches being used indoors with out the appropriate ground surface covering. Additionally the slide was broken in 2 spots and ducted taped to keep it together. The cubbies were not secure to the wall

The licensors observed the certified assistant and the children to be on a walk in the neighborhood the assistant did not have the first aid kit with her

The day of the visit there 4 children present. A review of the attendance records show that on 5/11/22 and 4/20/22 there were 7 children present on those days
Annual Registration in Professional Qualifications Registry (PQR)
The regular assistant was left alone to care for the children on Tuesday and Thursday mornings from approximately 7:30am-9:30am while the Educator left the premises to go to school. This was done from January-May and adds up to approximately 68 hours.
The licensor did not observe an opportunity for the children to engage in free choice of play due to the fact that there were very few toys/materials accessible for the children to choose from. The educator or the assistant chooses toys from another room for the children to play with
There is an person on the premises at least 1 time a week who has not completed a BRC and had that BRC approved by EEC. This person was on the premises alone with the children when the licensor arrived
The perimeter for the fall zone is not large enough. Discussed that there must be 6 feet on all sides of the swing set.

The licensor observed that there was not a distance of at least 2ft between the nap mats. The Educator moved the mats during the visit
The Educator has 2 children in care who require (REDACTED0. There was only 1 (REDACTED) on the premises and the original prescription label was not attached, instead the prescription label for two children was attached with a rubber band. The Educator was cited for this in February 2025.

The attendance records had not been completed since Thursday October 31, 2024. There was no attendance for Friday, November 1st or Monday, November 4th. The Educator completed the attendance for the day of the visit during the visit at the request of the licensor.
The Educator's license expired in June 2024. She has still not completed the required 30 hours of training to renew her license. The Educator has completed five hours at this time.

In the upstairs kitchen there were several drawers that were not locked and contained hazards: pizza cutter, peeler, cords, cleaning supplies and several bottles of prescription medication.

The licensor did not observe any evidence of curriculum/structure/routine. The children were observed to be watching TV and using ride on toys during the visit. This was observed for approximately 45 minutes. The children did not participate in outdoor play on the day of the visit. The certified assistant stated that earlier in the day the children colored, did math, stories and circle time

The parent handbook was missing the emergency plan
A review of the children's files show that 5 children need progress reports
On the emergency postings, the Educator and the assistant were listed as the emergency back up person.
The Educator's PQ expired 1/12/22 and the assistants PQ expired 9/15/21
A review of the children's records show that 4 children's general permissions were blank, 3 children did not have medical documentation, 2 children's medical did not have dates on them and 1 child's p.6 of the enrollment packet was blank
The Educator did not notify parents of a change in educators. There was an unlicensed caregiver caring for the children and parents were not notified. There is an issue with the children biting in the child care, the parents are not always notified when this happens verbally or written

The licensor observed the gate in the backyard that prevents the children from entering the area where the inground pool is was not locked. There was a only a stake in the ground on the other side of the gate
