The licensor observed that the gate for the fence was not in good repair. There was no lock on the gate so the educator had to use a chain to lock it.

A review of 6 records show that 2 general permissions need to be completed and 2 children need updated medical
The licensor observed the following in the outdoor space: a rusted metal pole, and fence pickets leaning against the fence, there was a shovel and a hoe accessible and there is chipping peeling paint around the bottom of the home. Additionally the opening to the fence is large enough to be considered an entrapment hazard. There is a hole in the ground that is being covered by a piece of grey wood.

As the children were going outside with the Educator she sent a 4 year old child inside to get her phone. The Educator was outside at the top of the stairs and could not see the child inside the home.

The licensor observed a (REDACTED) in a pack and play where the sheet was not tight fitted and the mattress did not fit in the pack and play correctly. The infant was just waking up from nap. This is the 2nd safe sleep violation
The licensor arrived at approximately 2:30pm. She observed 2 infants (REDACTED) to be awake in their pack and plays. The infants were not taken out of the pack and plays until 2:50pm.

The licensor observed the cord for the infant swing accessible and the cords for the dehumidifier accessible.

There are not enough developmentally appropriate toys/materials accessible to the children in the space. The licensor observed book, play doh, kinetic sand, markers, some puzzles, blocks and animals, a play kitchen with no accessories and some infant toys

The licensor did not observe the educator to wash her hands or the infants hands after diapering. Additionally the diaper table was not disinfected after each use

A review of the records show that 2 children were misisng a photo or physical description , 4 children’s general permission were blank and 1 was missing a signature. 1 child needs an annual update, 1 child needs an updated medical, 2 children need the emergency transporation form completed.
The first aid kit did not contain all the children enrolled’s emergency contact cards
The educator asked the licensor to watch the children while she went upstairs to get lunch. She stated she will usually have the children stand at the bottom of the stairs or watch them on the video camera on her phone while she goes upstairs to get lunch.
The stairs were not gated and children under 3 were present
The temperature in the fridge was 28 degrees
There was an exercise bike accessible to the children today.
The educator needs to add more toys/materials that are developmentallly appropriate for the children in care. The licensor observed a good variety of books for the children, there were some legoes and a little kitchen area and some cars/trucks, a drawing desk and some farm animals. The majority of the toys were for infants and there is currently [REDACTED] children in care.
