A review of four staff files, licensor noted the following: 1. One educator newly hired in ratio had an incomplete file missing all EEC pertinent information on the EEC checklist.
Staff meeting documentation were missing the months of April, August ,November and December 2024.
Staff orientation, training and meetings
Two children were missing consent forms for the medication.
Two emergency medications observed were not in the containers in which they were originally dispensed and with their original labels affixed.

Documentation of staff orientation was missing for all three staff files reviewed.
A review of the evacuation log found that it was missing the following months of 2022: February, March, May, June, and October. 2. Alternating exit routes were not being used.
A review of three childrens file found: 1: Three children's enrollment forms were not updated annually . 2. One child had an expired physical dated 7/14/2021.
One child had an IHCP form on file that was not signed and dated by the physician.
