A review of the first aid kit indicated that the kits did not have CPR mouth guards as required.
A review of four children's medication consent forms indicated the following: -Four did not have documentation of what happens if the medication is not administered. -Two did not have who was trained to administer the medication at the program.

The kitchen door was not locked. The kitchen stove was accessible to the children in the kitchen. The back door to the program was not locked. It leads to the church and then out into the parking lot.
The windows in the infant classroom were dirty and had water in between the two panes of windows.
The playground had several spaces that had unknown vegetation growing and accessible to the children.

Over the last two years, the program did not have documentation of staff meetings in August and September 2019.
There was a nail protruding slightly from the trim, in the gross motor room. There were stained ceiling tiles in several areas of the program.
One of toilets in the bathroom did not have a seat cover.
The posted menu did not include two components for snack time.
There was chipped paint on the half wall between the gross motor area and the kitchen.
