Children’s records were reviewed. It was observed that multiple records did not have date of admission documented, permission to take children off premises were not in children’s records, and permission to transport children to medical facility were not in children’s records, signature for acknowledging receiving copy of parent handbook were not completed. One file did not have medical physical in file.
Tooth brushing - Toothbrush Storage
In the kitchen it was observed there was access to plastic bags and cleaning products under the kitchen sink. Diapering table is one that can no longer be approved for use and must be replaced. Diapering table is not secure and is easily tippable.
Diapering table is easily moveable and needs to be secured.

Educator’s personalized parent handbook does not include all EEC required information for parents.
There was a person on premises that is not listed as a household member/regularly on the premises. Educator indicated that this person present today only to help with her (REDACTED).
Educator did not have progress reports for two of the children that are now due for a progress report.
Educator’s assistant, Oscar Aranda, is not signing in and out daily as required to verify attendance.
It was observed that two toothbrushes were not observed to be stored in a sanitary manner. One was in a ziplock back and the other was open and in a container that had other items that were not for tooth rushing.
It was observed that there was (REDACTED) napping in her pack and play. It was observed that sheet on pack and play mattress was loosely fitting. There was a diapering table attached to pack and play that contained a variety of items, such as extra clothing, diaper cream, and a toy. Safe sleep regulations and policy were reviewed with educator.
It was observed that the educator did not wash her hands or the infant’s hands after diapering.
