Three children were missing the consent form for the medications.
Two children on two emergency medications were both missing the IHCP for one of the medications
1. One child emergency medication expired 11/24. 2. One child prescription medications was not stored in the containers in which they were originally dispensed and with their original labels affixed.

One staff had a physical examination dated 1/12/22.
The evacuation log was missing the month of December.
This provisional license 90-day monitoring visit inspection recorded no violations or advisories.
Accurate attendance; arrival/departure times
A review of the attendance roster found that on 11/9 there were 2 children that were not signed out.
On the day of the visit three children were observed being supervised by an assistant group leader. Assistant group leader may not be responsible for a group of children. EEC observed a staff member with the children. When the PD was asked about the staff member EEC was informed that the staff member had not been hired yet. EEc determined the staff member did have a current BRC. Only staff members who are in hired status can be with children.
Past attendance sheets were reviewed upon arrival and these sheets indicated that children were marked present upon arrival and departure. On the day of the visit, children were not immediately accounted for on attendance upon arrival. Children's arrival and departure time must be noted daily.

A review of three children's record found that: 1. One child did not have a current physical examination and immunization record 2. One child had an expired physical dated 7/ 2021 and now is over a year old.
A review of three staff files found that there was no evidence found that: 1. no evidence of the date of interview 2, No evidence of 2 references 3. No proof of staff orientation in any files.
The staff schedule was not posted.
One child on a rescue medication had an Individual health Care Plan form (IHCP) form that was not signed or dated by the doctor.
