A review of five children’s records show that: 2 children require an annual update.
The licensor observed the following hazards: bathroom; there was hand sanitizer, and several bottles of oils on the counter. Next to the counter there was a box with perfumers and hair products accessible. In the kitchen the cabinet under the sink was not locked and there were cleaning supplies accessible and one drawer was not locked and scissors and a knife were accessible.

The licensor observed several times today when there was not appropriate supervision: while the children were in the playroom the licensor observed several of them climbing on top of a shelf and sliding off or jumping off. The assistant was sitting in a chair next to the shelf during some of this time. One child was (REDACTED) 2 times and it was not addressed by the educators. One toddler was observed to come out of the playroom, go behind the educator and be in the kitchen trying to open the door for approximately 1 minute before the educator went to get her. In the backyard the children were running with sticks and one child (REDACTED) with the stick. On the deck, 1 child climbed on the bench and was leaning over the railing on 2 occasions. Additionally one toddler was observed by the licensor to be sitting on the top of a slide outdoors that is not approved for use.

There were 2 different occasions during the 2/20/2020 visit when the educator was in the kitchen and the licensor observed a child to be standing on the table in the playroom. This same child was observed to be climbing on the table during the visit on 1/18/2020. The educator stated that this child had been climbing on the table recently. The educator did not provide appropriate supervision to this child when she left the room and was unable to see the child.
Although the educator is documenting the arrival/departure times for the children and it was up to date, she did not have a method to know who exactly is on the premises at any given point in the day There was a school age child on the premises that the educator was unaware of . Additionally there was a household member on the premises that the educator was unsure if they were present or not.

The following hazards were observed in the backyard: an uncovered grill, there was a large pile of sticks accessible to the children, and a large hole that the educator tried to cover up with bricks, one child was observed to get his ride on toy stuck in the hole. The educator covered the hole with dirt while the licensor was present
