A review of 9 children records show that 4 children need their general permission completed two children need a annual update, three kids needs proof of lead test, one child needs and updated physical.

During the visit it was observed a pile of plastic bag next to changing area . Educator was able to remove the bags after licensor explained that they are a hazards for the kids . It was also noticed that the diaper table was not secured on the wall which is a hazard. Licensor also observed In the bathroom there were laundry detergents cups at reach to the kids .In addition, it was observed two slides were on the cement outside.
Progress reports are not being completed on a regular basis.

There was no gate preventing the children from accessing the stairs by the office space leading upstairs and all the children present were under the age of 3
