At the time of the visit (2) educators did not complete the initial EEC Strong Start Essential trainings.
At the time of the visit several of the children's records and children record checklist are expired.
At the time of the visit (1) educator had an expired First Aide certification.
At the time of the visit during the health and safety walk through a couple of the educators were not supervising the children as their attention was diverted to other task.
At the time of the visit the educator schedule was posted however it was not current.
At the time of the visit EEC observed the following: an educator had an older infant in their crib for an extended period of time not interacting with them and when the educator went to place the older infant in the crib they were not as gentle with placing the child on their back.
At the time of the visit the program was not handling the school's lunch in a sanitary manner as the carrots were in a strainer in the sink and the meat was in a container in the sink.

At the time of the visit the program had the following hazards within reach of the children: a couple of educator purses, wires longer than 6 inches long, and a roll of plastic bags in the bathroom.

At the time of the visit the program had several children with challenging behaviors and the program did not create a behavior intervention plan between home and school.
At the time of the visit the program did not have all of the required EEC documentation in all of the educators file. The following documentation was missing: interviews, orientation, (2) references, orientation, evidence of qualifications, work verification, and EEC training. The program did not have a current Staff Record Checklist.

It was observed on the playground during the visit a {REDACTED} educator become frustrated with a child when they pointed their finger in the child's face then proceeded to move the child by their arms to a time out on the bench. At the time of the visit the program was not in ratio in the {REDACTED} classroom as there were 3 educators (1 teacher qualified and 2 assistants) with 14 children and the EEC ratio is 2 educators with 9 children.
At the time of the visit the children were not assigned to a specific group with consistent staffing as there were several educators who called out.
Due to the educator call outs and the program was over ratio and there was no evidence of curriculum happening within the two operating classrooms.

There is a pattern of the classrooms being over ratio with a variety of time ranging from 10 minutes to several hours. The ratio is 2:7 for {REDACTED} with the program having 2:10 {REDACTED} and the ratio for {REDACTED} is 2:9 with the program having a max of 2:15 on occasion for 10- 15 minutes. This was citied on previous visits 9/29/22 and 11/3/22.

At the time of the visit the program is not discussing behavior management techniques among the staff to promote consistency with child guidance and interactions.
At the time of the visit the program was transitioning the children to other classrooms without parental permission.
At the time of the visit the program is not following their Referral Policy to ensure the children are being referred for the appropriate assistance to be successful in the program.
At the time of the visit the program was utilizing several educators as qualified teachers when they do not have the specific requirements to be in this role.
At the time of the visit the program did not have evidence of conducting and documenting the month of February.
At the time of the visit the program had (1) expired Epi pen.
At the time of the visit the program did not following their handwashing policy as two children did not wash their hands after using the bathroom.

At the time of the visit it was observed an educator used a harsh tone when interacting with a child on the playground.

At the time of the visit one educator was not able to supervise all of the children in the classroom as they were assisting a couple of children in the bathroom.
In the {REDACTED}, {REDACTED}, and {REDACTED} classroom the educators are grabbing the children by the arm to move them and no interactions in the {REDACTED} classroom.
In the {REDACTED} classroom the educators did not have a routine or curriculum to implement with the children.
At the time of the visit the following was not secured: several of the radios and (1) drying rack were on the classroom shelves not secured and able to tip over on the children.
At the time of the visit the {REDACTED} classroom had diaper cream accessible and within reach of the children in the bathroom.
The program did not have (1) medication with a box or the prescription adhered.
In the {REDACTED} classroom a child drank from both water bottles without the educator noticing and the educator gave another child one of the water bottles to drink.
At the time of the visit the children were not being supervised by the educators as two of the {REDACTED} children were eating things off of the playground ground, an {REDACTED} child was eating paper from the floor in the {REDACTED} classroom, and a {REDACTED} child was crawling on the classroom tables licking the crack between the two tables.
The program classrooms are not consistently implementing curriculum and engaging activities with the children.
One of the cribs in the {REDACTED} classroom was not being used as it is intended as the program was storing a mat, bouncy seat, and another type of bouncy for the children in the crib.
The {REDACTED} educator did not wash an infant's hands prior to the {REDACTED} eating their finger food.
At the time of the visit EEC reviewed the Staff Record and Staff Record checklist noting several of the EEC required documents are missing from the file.

At the time of the visit the program did not have evidence of curriculum as cited at the previous visit.
In the {REDACTED} classroom the hook for the first aid bag was not secured as the three screws were sticking out and one of the cabinets had a broken door.
At the time of the visit the educators did not wash their hands or the children's hands prior to eating snack, the educator did not wash their hands in between changing the children's diapers, and after wiping a child's nose as cited at the previous visit. One educator used the floor broom to sweep up the toys not ensuring the toys are cleaned as the broom is used to clean the floors.

The educators in the {REDACTED} classroom did not have an understanding of the individual needs and development of the children as they were not familiar with the children and schedule in the classroom.
