The program did not have a current Building Inspection Certificate available for review at the time of the visit.
Upon a review of five children's Individual Health Care Plans, the following was identified: -Two children's records did not have a current and complete Individual Health Care Plan (IHCP).
Upon a review of five children's Medication Consent Forms, the following was identified: -Five children's records did not contain a Medication Consent Form for each prescribed medication.
Staff BRC - R Sep 2025
Upon a review of the BRC Navigator, all staff with the exception of one are overdue for a background record check renewal. The program's Staff Record Checklist indicates that staff were listed as suitable between the months of June-September of 2022 but this is not reflected in the BRC Navigator.
A current building certificate was not available on the day of the visit. The certificate at the program expired 3/12/2020.
A review of three children's records indicated; a child with a chronic medical condition listed in their record did not have a Individual Health Care Plan.

Knowing who assigned to supervised
Two educators indicated that they were assigned to supervise 20 children in the upstairs room. The attendance record and physical count indicated that 22 children were actually present.
