Staff and children wash hands: after toileting or diapering
One child's Enrollment Form, Transportation Form, Emergency Medical Consent Form and Developmental History Form were expired.
During the visit, three staff files were reviewed. One file did not include teacher certification or college transcripts and documented work experience to determine their qualification status. One file did not include evidence of Program Orientation.
The attendance was inaccurate in the Toddler 1 Classroom. There were 9 children observed in the classroom yet, there were 10 children signed into the classroom.
One child's Individual Health Care Plan included the administration of Benadryl. However, the program did not have the Benadryl on the premises. Therefore it was not immediately available if needed.

In the Infant 1 Classroom, a feeding bottle was observed on a shelf and was not labeled with the name of the child it was intended for.
One child with emergency medication on the premises did not have a Medication Consent Form signed by their parent.
In the preschool bathroom, toys and plastic totes were stored under the sinks. The items were stored in a manner that prevented the children from easily using the sinks to wash their hands. The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter.
One child's Individual Health Care Plan was incomplete. The plan did not include who has been trained and will be administering this treatment while the child is at the program.
