Napping children in several of the rooms were less than 2 feet apart form one another.
At the time of the visit one child's emergency medication was not in its original packaging.

At the time of the visit three children with emergency medications were reviewed and the following was found: one child had no medication consents for their prescribed medications and one child's medication consent was missing a parent signature..
Diapering and toileting policies/procedures were not posted in diaper changing areas and child bathrooms throughout the program.

The exit at the back of the building did not have proper evacuation procedures posted at it.
-A review of three staff records indicated that two educators did not have evidence of current physical examinations. -A review of the Staff Records Checklist indicated that bi-monthly staff observations had not been completed for the duration of the 2021-2022 school year. Please submit a plan that includes how these will be completed moving forward.
The following was observed and accessible to children: (Suite 104) BATHROOM -The trash containers were not covered. KITCHEN -The kitchen drawers were unlocked and contained trash bags and a can opener. INFANT -A bottle warming appliance was being used to heat a bottle. The educator was informed of the policy and immediately stopped using the warmer. -A music player cord was draped behind a crib where a child was sleeping. This was removed during the visit. TODDLER 1 -A broom and dustpan were propped against the wall. TODDLER 2 -The surface of the pad on the changing table had sand. -A play kitchen was not secure. PRESCHOOL 1 -Three cords were draped within children's reach. -Two toy storage units were tippable. -The broom and dustpan were propped against the wall. (Suit 107 & 108) BATHROOM -The trash containers were not covered. -There was a metal paper towel holder with sharp edges in reach of children. PRESCHOOL 1 -A play doll house was not secure.
A review of the medications at the program indicated that five children did not have medication consent forms as required.

Documentation of medication administration; Current consent for medications
The following was observed during the visit: (Suite 104) PRESCHOOL 2 -There were 10 children in the classroom but 9 were marked as present on the attendance list. TODDLER 1 -There was an (REDACTED) child present. The educators stated that the child was transitioning into the toddler group. The toddler age group's minimum age is 15 months. INFANT -There were 4 infant children present with 1 educator. A second educator returned to the classroom a few minutes later. Technical assistance was provided regarding child to staff ratios.
The program had sleeping bags available to toddler and preschool children for rest. The Licensee must provide a separate sleeping surface for each child present at any time during the day.
(Suite 104) The changing table in Preschool 1 had chipping and peeling paint.
A review of three children's records indicated that three Individual Health Care Plans (IHCP) were incomplete. The plans did not include who has been trained and will be administering the treatment while the child is at the program.
