Current consent for administered medications
At the time of the visit, in a review of three students IHCPs and the corresponding Medication Consent documentation, the documentation was not fully complete. Student B was prescribed two medications. One medication consent did not have documentation on if the student had taken the prescribed medication previously. All medications must be accompanied by written parental consent and healthcare practitioner authorization, as applicable.
At the time of the visit, in a review of three educators files, the documentation was not fully complete. Educator C had documentation of one MMR but was missing documentation of their second MMR. The program must maintain personnel records as outlined by the staff record checklist.

Emergency medications readily available
At the time of the visit, in a review of two students IHCP's, one student did not have a doctor provided IHCP for their diagnosed (REDACTED).
At the time of the visit, in a review of two students medication consents, one medication consent did not have the side effects and directions for storage listed. Another student had two prescribed medications listed on one medication consent. Each medication needs its own, fully completed, medication consent form.

At the time of the visit, the preschool classroom was outside on the playground. There were 12 students with one educator and one volunteer. The second educator assigned to the group remained in the classroom for a period of roughly 10 minutes. The program must have the number of educators necessary to ensure adequate supervision of the group at all times.
There was not a current Parental consent form for administering medication for a child with a peanut allergy; the dr. orders stated that the child is prescribed an epi pen and antihistamine in the case of an emergency. In addition, the program did not have parental supplied antihistamine available.
There was not an Individual Health Care Plan for one child with asthma, and for one child with a peanut allergy.

At the time of the visit, emergency medications remained in the classroom while the students were outside. Emergency or life-saving medications, such as asthma inhalers and epinephrine auto-injectors, must be readily available for any children for whom they have been prescribed.
