At the time of the visit, in a review of the fire drill log, documentation of the route taken was missing for the drills conducted January-May 2026. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.

At the time of the visit, it was observed that one educator left the 'First Playground' and walked inside. This left one educator with eight toddler/preschool students outside. The program must have the number of educators necessary to: ensure adequate supervision of the group at all times.

At the time of the visit, the following was observed indoors: Within the PTR Classroom: the trash was missing a cover and there was a spray bottle with clear liquid, unlabeled, within reach of children. Within the PS2 classroom: the trash was half covered and there was a toilet plunger accessible to children. Within the bathroom at the end of the hall, used by the KReady students, there was a toilet brush and two plungers accessible to children. The indoor space must be clean and safely maintained. Garbage must be kept in lined and covered containers. All toxic substances must be labeled as to the contents and antidote. At the time of the visit, the following was observed outdoors: on the 'first playground' there was a slide pushed against the fence and did not have a use zone of six feet in all directions. The use zone should extend a minimum of 6 feet in all directions from the perimeter of a stand alone climber.

At the time of the visit, in a review of four students files, the information was not fully complete. Two students had expired progress reports. One student was missing documentation of a progress report and a lead screening. Children's records must have all required applicable documentation according to the Children's Record Checklist.
Emergency Evacuation
At the time of the visit it was observed that there was chipping and peeling paint on the Exit doors within the Infant Two and TTR Classrooms. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.
At the time of the visit, it was observed that students with the KReady classroom were permitted to use the bathroom across the hall from the classroom. At the time of this observation, one educator was standing in the doorway of the classroom, responsible for supervision of the student in the bathroom and the KReady classroom. Educators must exercise appropriate supervision of the children in their care. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.
At the time of the visit it was observed that an allergy list was posted, however it was not current. A list of allergies and/or other emergency medical information provided by the parent for each child; and a list of all emergency or life saving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications, that specifies to which children they belong; must be posted in a manner that protects the privacy of each child.

Toxic Substances
Infant one and Infant two classrooms did not have accurate attendance of the children within their care; one child in each classroom was not signed in.
A review of four IHCP's it was observed that medication consent from the parents were missing for all four.
During a review of the emergency evacuation drill log book, it was observed that drills were not recorded in April, November or December of 2023 and January and February of 2024. The director stated that the classrooms log their own drills.

In the preschool one classroom there was an open bag of grass seed on the floor accessible to children. The warning label stated to keep out of reach of children.

In Infant 2, it was observed that an infant was laying down while drinking a bottle. Infants must be held while fed a bottle.
It was observed that in Preschool 2, the wall in the Dramatic Play area had chipped paint.
A review of three children's records indicated that two were not updated annually.
A review of the injury/incident log indicated that incidents that occurred between January 21-26 were not documented on the log. Individual incident reports were located in the child's record.
It was observed that in the PTR classroom, there wasn't a cover on the trash container. Inside were soiled diapers.

A review of the attendance record in Toddler classroom was not accurate. There were six children signed in and seven present. When discussed with the Educator in the classroom, they stated that children need to be signed on both on the classroom sheet and ProCare system.
A review of the Emergency Evacuation log indicated that an evacuation drill was not conducted monthly. The Program Director reported that it has been difficult to schedule.

It was observed that in the Toddler Transition classroom (TTR) kitchen area, there were cereal bags not stored in a covered container.
A review of three Individual Health Care Plans (IHCPs) indicated the following: -One was not fully completed to include which staff were trained and who were they trained by. In addition, the three IHCPs were not accompanied by a current medication consent form.
A review of three children's files indicated that ll three did not have an evidence of an annual parental update of enrollment information.

At the time of the visit, in a review of three educators files, the documentation was not fully complete. One educator was missing documentation of PQR. One educator had an expired PQR and was missing documentation of two MMRs and an annual review. Another educator was missing documentation of two references and program orientation. The program must maintain a personnel record for each staff member.
At the time of the visit, it was observed that a Staff Schedule was not posted. It was observed that the Assistant Director was working on an updated staff schedule, that was not fully complete. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.
At the time of the visit, in a review of the Emergency Drill Log, documentation of all drills from 2024, January 2025 and February 2025 were unable to be located. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
At the time of the visit, it was observed that one Inhaler and antihistamine prescribed on multiple students IHCPs were stored in a cabinet within a room attached to the front office. The medications were not readily available. Emergency or life-saving medications for any children for whom they have been prescribed must be readily available.

During a review of four children's files, it was observed that two files were missing documentation of past progress reports.
The PTR classroom did not have a thermometer in the refrigerator and the K-Ready classroom had a refrigerator that was reading at 42 degrees.
The following was observed: The preschool one classroom had rust on the sides of the sink of the classroom. The PTR Classroom had a trash used within the bathroom did not have a lid, there were water stains on the ceiling on the classroom, and an open box of plastic diaper bags was accessible .
The upper and lower playgrounds were observed to have two "wooden mud kitchens" that were not secure to the fence. The lower playground was observed to have two large buckets with cracks in them and a sand box cover had a large crack within it. A basket ball hoop on the lower playground was not secured. The lower playground had large holes along the left side of the fence that could be potential tripping hazards. The lower playground had a slide without proper surfacing under it. The infant playground had rocks within the grass that could be potential choking hazards. The director stated that the program brings out blankets for the children to lay and play on.
The preschool two classroom had chipping paint on the walls., the PTR classroom has paint chipping on the exit door.
It was observed that within the Toddler classroom, an evacuation route was not posted by the door that leads out of the building within the classroom.
A review of the evacuation drill log was indicated that a drill was not conducted consecutively on a monthly basis.
A review of eight children's medical records indicated the following: -Three individual health care plans were not fully complete with all the necessary information. -One individual health care plan was expired as of 3/23.

A review of three staff records indicated that one did not have a signed orientation checklist and two did not have evidence of bi-monthly observations.
It was observed that there was wood chip nuggets on the Toddler outdoor play area. Pea gravel and wood chip nuggets must not be used in areas used by infants and toddlers.
It was observed that a snow shovel and other equipment was accessible to children next to the water hose alongside the building facing the playground.
A review of the Evacuation drill log indicated that there wasn't a drill conducted during the months of January 2021, February 2021 and January 2022. In addition, the total number of children was not documented.
A review of three children's medication documentation indicated that all three did not have evidence of current written parental consent.
A review of three Individual Health Care Plans (IHCP) indicated that one had expired as of 4/29/2022.
The building inspection expired as of 7/1/2021. The Licensee stated that they are actively searching for a contractor to make repairs in order to renew their building inspection certificate.
The following was observed and accessible to children: TODDLERS - There was a broom and dustpan located next to the trash container and changing table; children were observed trying to touch them. There was a staff purse and metal drinking bottle located next to the classroom door. A child was observed picking up and dropping the metal bottle on the floor. PRESCHOOL 1 - There were bags and other materials in front of the secondary exit outdoors. Exits and evacuation routes must be kept clear of obstructions. PRESCHOOL 2 - There was a broom and dustpan located next to the children's cubbies.
A review of three medications indicated that one was not stored in the original box with a prescription label attached.

In the Preschool Transition classroom (PTR), an educator did not use a positive tone when redirecting a child and was overheard telling a child “no to you!”
A review of Emergency Evacuations indicated that a drill was not conducted in the month of December 2019. When discussed, the Director stated that it was “too rainy and too cold”.
