Current consent for administered medications
At the time of the visit, in a review of two students IHCPs and the corresponding medication consent documentation, the documentation was not fully complete. One student was missing documentation on if they had previously taken the medication.
At the time of the visit, in a review of two students IHCPs the documentation was not fully complete. One student was missing documentation of the potential side effects of their prescribed medication. The second student was missing documentation of their diagnosis from a medical provider. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

At the time of the visit, in a review of four educators files, Educator D was missing documentation of bi-monthly observations. The program must maintain a personnel record for each staff member as outlined by the staff record checklist.
At the time of the visit, seven students were signed into the attendance but nine students were present. The Director was able to quickly identify who was not signed in, and updated the attendance record. The program must maintain daily attendance records indicating each child's attendance, including arrival and departure times.
At the time of the visit, in a review of the emergency drill log, documentation of March, April and May 2025 drills were missing. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
At the time of the visit, it was observed that the Lead Teacher on site was walking into a back room separate from the classroom to get various supplies. During this time, an Assistant Teacher was alone in ratio. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.
Supervision of Children
At the time of the visit, the Director was not on site. The EEC qualifications of the individual on site identified as the qualified person designated in charge of the program in the Directors absence were unable to be confirmed.
At the time of the visit, in a review of three staff files, the information was not fully complete. One educator had an expired PQR, two educators were missing documentation of staff observations. The program must maintain a personnel record for each staff member.

The following was observed to be accessible to children: The two gray bookshelves that were previously cited during the last visit on 1/23/2024 (#141423) were not secure today. The safety straps attached to them were not effective. The Corrective Action Plan submitted by the program stated: The 2 grey bookshelves were secured to the wall on1/24/24.
The Building Inspection is not current and expired on 12/31/2023. The quarterly Fire Inspection is not current and was last conducted on 6/1/2023.

One child's EpiPen was not in the original container with the label from the pharmacist.
One of the two educators stated none of the children had medication when in fact two of the children have emergency life saving medication.
The following was found to be accessible to children: -Two gray bookshelves were not secure and tippable. One was used to store pillows and the other was used to store cardboard blocks. -The Fire Extinguisher on the wall in the right hand front corner of the room is rusting along the bottom and has chipping red paint. -In the bathroom, a plunger was sitting next to the toilet and two boxes of opened gloves were sitting on a low shelf. -The top electrical outlet on the wall across from the bathroom was not covered. -The door from the classroom to the garage was unlocked and the garage contains various cleaning products on shelves and electric tools. -The door to the nurse's office was open and the following was found: a tippable plastic shelving unit held a can of aerosol Lysol spray with the label "Keep out of reach of children", loose trash bags, an open box of sandwich bags and an open box of gloves. All loose bags are suffocation hazards and the gloves are a choking hazard.

At the time of the visit, it was observed that the preschool students were using a playground that had not been assessed by EEC. The various structures/equipment was later identified as being designed for the age groups 2-5 years and 5-12 years. During the visit, it was observed that preschool students were using equipment designed for children 5-12 years old.
At the time of the visit, in a review of three students prescribed medication, two students had three different prescribed medications that were expired. One student had an inhaler that was not in its original box and was missing the prescription label. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging. Medications must be current and not expired.
At the time of the visit, in a review of three students IHCPs the documentation was not fully completed and/or current. All documentation was missing the educators trained on each health condition. One student had two prescribed medications, both medication consent documentation was missing if the student had taken the medication before. One students IHCP and corresponding medication documentation was expired. One student did not have a complete IHCP provided by their medical provider. The IHCP document was missing the name and description of the health condition and its symptoms. One IHCP was not signed by the parent.

The Staff Records Checklist is missing the new staff member.
The program does not have a Child's Record Checklist. One child's record is missing evidence that a lead test was completed.
The IHCPs were reviewed and found both were incomplete. They were missing who is trained to administer the medication and who trained them.
Previously cited at the last visit on 1/23/2024: One child's EpiPen was not in the original container with the label from the pharmacist. Corrective Action Plan submitted: Family has been asked to either get a new medication in the original box, or get a copy of the prescription label for the epi pen that they have. Visit 2/21/2024: The child's EpiPen is not in the original packaging with the label from the pharmacist.
The medication consent forms were reviewed and found that parents had not signed the forms.

Eleven child records were reviewed and found: -One child record did not have a current physical (last physical dated 9/23/20222), did not have an IHCP, allergy plan, or medication consent forms. -One child's record did not have an IHCP or medication consent forms. -One child's record was missing evidence of a lead test. -One child's record did not have a physical or immunizations.
The medical alerts for children were not posted. The list was in the emergency bag.
The Staff Schedule posted was not current. An educator listed no longer works at the program and one of the educators present during the visit was not on the schedule.
Upon review of the children's records, it was discovered one child has an allergy and needs an EpiPen and antihistamine. These medications were not available during the visit.
