The outdoor space did not have shade. A structure was set up over the sand box, however, the cover was removed. The outdoor play area must provide for both direct sunlight and shade.
One staff hired in February 2023 did not have evidence of completion of First Aid certification.

A review of three staff records indicated that one record did not have evidence of a current physical examination.
A review of the evacuation drill log indicated that a drill was not conducted during October 2022.
A review of one child's individual health care plan indicated that it did not identify which staff were designated to administer the medication.
The following were observed: There were bolts protruding form the brown and black borders, there was a cracked corner edge on the sensory table cover, a bottom corner of the metal fence was raised and curled.
The following were observed and accessible to children: FRONT ENTRANCE - To the left of the front door, there were containers of disinfectant wipes with a warning label to "keep out of reach of children". TODDLER/PRESCHOOL - On top of the changing table, there was a disinfectant spray bottle and a box of gloves within reach. Underneath the changing table in an unlocked cabinet, were plastic bags.

One staff hired in February 2023 did not have evidence of completion of CPR certification.
Four child records were reviewed and found: -Two children's records were missing Individual Health Care Plans and medication consent forms. -One child's record did not have current a enrollment form, physical, developmental history, consent for CPR and First Aid and transportation plan. They all expired 9/27/2023. -One child's record was missing an Individual Health Care Plan and medication consent form and did not have current CPR/First Aid consent and transportation plan (both expired 9/20/2023).
Three staff records were reviewed and found: -One staff record was missing evidence of MMR.
The following was observed: -In INFANTS, two pacifiers and one water bottle in TODDLER 1 were not labeled with children's names. The water bottle was labeled during the visit.
The following was observed to be accessible to children: -In TODDLER 1, the back side of the changing table is peeling. -In PRESCHOOL/SCHOOL AGE, an open box of gloves was sitting on a low shelf. The box of gloves was moved during the visit. -In PRESCHOOL, an open box of gloves was on a low shelf and a bucket of antibacterial wipes sat on the edge of a shelf at the level of the children's' heads. The gloves and wipes were moved during the visit. -In the PRESCHOOL BATHROOM, a floor tile in front of the trash can was broken.
The Building Inspection is current (expires 12/31/2023) but the current Fire Inspection certificate has not been obtained.
The following was observed to be accessible to children: -In PRESCHOOL, the paint on the bottom of the math wall was peeling off.
Medications were reviewed and found one inhaler did not have a box.
