At the time of the visit, in a review of two students IHCPs and the corresponding medical documentation, the information was not fully complete. One student was prescribed two medications for one diagnosed condition. Both documents were missing documentation of the times the medication was to be provided and the directions for storage. One medication consent was missing documentation of the side effect of the medication,
At the time of the visit, it was observed that the woodchips on the playground were not at adequate depth. The woodchip depth ranged from roughly half an inch to 2.5 inches. The outdoor space must be properly maintained.
At the time of the visit, it was observed that students were permitted to use the bathroom without proper supervision. It was observed that the program was permitting one student at a time to leave the cafeteria to use the bathroom down the hall. When an additional student requested to use the bathroom, an educator would then position themself in the hall to supervise. Educators must always be positioned to maximize their ability to see and/or hear children in their care.

At the time of the visit, in a review of three educators files, the information was not fully complete. Educator A was missing documentation of current Professional Development. Educator B had an expired PQR and expired Staff Observations. The program must maintain a personnel record for each staff member as outlined by the staff record checklist.
At the time of the visit it was identified that one educator on site had an expired BRC. The employees BRC had been expired for two months. The licensee must maintain in each personnel record: evidence of completed background record checks, as required by 102 CMR 1.05(2) and 606 CMR 14.00.
At the time of the visit and in a review of the programs document library, the Building Inspection and Fire Inspection were expired. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code. Every program not located in a residence must submit evidence of compliance with applicable fire codes.
At the time of the visit, in a review of two students IHCPs, one student had an expired IHCP. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
In a review of three educators files, all three were missing documentation of annual evaluations and documentation of current professional development hours. One file had an expired physical.

The amount of woodchips on the playground are not at an adequate depth at the fall zones. During the visit the woodchips at the fall zones were roughly two inches deep.
In a review of three student's files, one student had one medication consent completed, listing two medications on the medication consent, and had three medications listed on their IHCP. Please complete one medication consent form for each medication prescribed to the student.
A staff member's purse was accessible to children; this was moved at the time of the visit.

A review of the program's evacuation drill log indicates that the "route taken" is not documented as required.
Attendance Recording and Group Sizes/Ratios
The following was observed during the visit: -One child was walking around the cafeteria with scissors. -Two children were wrestling on the rug. -A group of children were observed playing inappropriately with a plastic tool box. -One child walked to the bathroom without notifying a staff member. -Two educators had their back turned on a large group of children and were eating their lunch. One educator was using their cell phone. -Staff stated that during half day programs the school does not allow the program to use any other space. -Educators were not exercising appropriate supervision of children. Please submit a supervision plan for half days at the program.

On the day of the visit, there were four staff assigned to supervise 40 children. The maximum group size for school age children is 26 children.
