Current consent for administered medications
At the time of the visit in a review of two students IHCPs and the corresponding medical documentation, the medication consent documentation was not fully complete. Both students were missing documentation of the dosage of the medication. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required, including proper administration of medication.
At the time of the visit, in a review of two IHCPs and the corresponding medication, one student did not have their life saving medication on site. The parent was contacted by the Site Coordinator, and the parent dropped off the life saving medication during EECs visit to the program. Emergency or life-saving medications, such as asthma inhalers and epinephrine auto-injectors, for any children for whom they have been prescribed must be readily available.
At the time of the visit, in a review of three employees files, the information was not fully complete. Employee A was missing documentation of an annual evaluation and had expired staff observations. Employee B was missing documentation of application/resume, documentation of interview, verification of two references, documentation of a physical, program orientation and staff observations. The Program must maintain a personnel record for each staff member, as outlined by the staff record checklist.

At the time of the visit it was identified that one educator on site had an expired BRC. The employees BRC had been expired for nine months. The licensee must maintain in each personnel record: evidence of completed background record checks, as required by 102 CMR 1.05(2) and 606 CMR 14.00.
At the time of the visit the following was observed: An Assistant Leader left ratio to throw something away outside, this left the Site Coordinator alone in the cafeteria with fifteen students. At one point, the Site Coordinator stepped out of the cafeteria, behind closed doors, into the hallway to make a phone call. This left the uncertified Assistant Leader alone with fifteen students. Educators must be positioned to maximize their ability to see and/or hear children in their care. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.
At the time of the visit and in a review of the programs document library, the Building Inspection and Fire Inspection were expired. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code. Every program not located in a residence must submit evidence of compliance with applicable fire codes.
At the time of the visit, it was observed that the woodchips on the playground were not at adequate depth around the fall zone areas. The outdoor space must be properly maintained.
At the time of the visit in a review of two students IHCPs, the documentation was not fully complete. Both students were missing doctor provided IHCPs. The EEC IHCPs were completed for both students, but were not signed or dated by either student's parent. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

In a review of three educators files, all three were missing documentation of a current physical and documentation of current professional development. Two educators files were missing documentation of the educators date of birth, annual evaluations, current PQR and staff observations. One educator's file was missing documentation of MMR, program orientation, and documentation of completing EEC Essentials.

During the visit, two staff files were reviewed. One file did not include a Verification of School Age Work Experience form. Two files did not include evidence of the required professional development hours for 2018. Staff observations were not conducted on a bi-monthly basis as required. One file included Background Record Check results. This information must be stored in a separate locked file.

The location of the first aid kit was not posted in the cafeteria.
During the visit, the following issues were observed. A moving skit, cardboard boxes, empty trash bags and debris were located at the entrance to the program and must be removed. The hardwired EXIT sign located above the program entrance door is hanging by the wires. In the Boy's bathroom, there are several vent covers missing or broken. The bottom of the stalls are rusty. In the Girl's bathroom, one vent cover is missing.
One child’s emergency medication has expired.
In a review of three educator's files, two were missing documentation of current First Aid Training.
The refrigerator on site did not have a thermometer.
In a review of three students files, two students did not have IHCP's or Medication Consent forms for their diagnosed health conditions. Both students had more than one diagnosed health condition.
