Current consent for administered medications
At the time of the visit, the evacuation procedures were missing from the gym, at the door leading into the hallway. Written emergency/ evacuation procedures must be posted next to each exit.
At the time of the visit, in a review of four educators files, the documentation was not fully complete. All educators were missing documentation of Program Orientation that meets EEC Requirements. Three educators were missing Work Experience forms. Educator A had an expired physical and an expired annual evaluation. Educator B had expired First Aid and CPR documentation. Educator C was missing documentation of their second reference. The program must maintain a personnel record for reach staff member as outlined by the Staff Record Checklist.
At the time of the visit, one student was permitted to walk into the back office area, unsupervised, on two separate occasions. To get to the back office the student left the cafeteria space, walked through a storage closet and into the back office. The student was unsupervised for roughly 10-15 seconds on each occasion. Educators must be positioned to maximize their ability to see and/or hear children in their care. The storage closet and the back office are not licensed EEC spaces for students.

At the time of the visit, the proper documentation was not in file for the 'Site Coordinator'. It was unable to be determined by EEC if the employee met the criteria for the program administrator. The licensee must ensure that the program is soundly administered by qualified persons designated with specific administrative and program responsibilities.
In a review of the updated SRCL emailed to EEC by the Site Coordinator, one educator had an expired BRC. The BRC expired on 12/19/2023. The educator was not on site at the time of EEC's the visit to the program and the Licensee identified the educator as a part time employee. The licensee must maintain in each personnel record: evidence of completed background record checks, as required by 102 CMR 1.05(2) and 606 CMR 14.00.
At the time of the visit, in a review of four students IHCP documentation, the documentation was not fully complete. Student D was missing documentation of potential consequences if medication was not provided. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
At the time of the visit, in a review of four IHCPs and the corresponding Medication Consent Documentation, the documentation was not fully complete. Student D was prescribed two medications for their diagnosed health condition. Both of their medication consents were missing documentation on if they had previously taken the prescribed medication. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required.
At the time of the visit, emergency medication for a student present in the program was stored in the back office and was not readily available. To get to the back office, an educator would need to leave the cafeteria space, walk through a storage closet and into the back office. Emergency or life-saving medications, such as asthma inhalers and epinephrine auto-injectors, for any children for whom they have been prescribed must be readily available.
This investigation inspection recorded no violations or advisories.

At the time of the visit, in a review of two students IHCP's and their corresponding medications, one student did not have their medication stored appropriately. The student did not have their inhaler in the original box with the prescription label attached. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging.
At the time of the visit, in a review of two IHCPs and the corresponding documentation, one student had a EEC Medication Consent form that was not signed by their parent.

A review of the Staff Record’s Checklist indicated: -Two files did not have evidence of immunity to MMR. -One file did not have evidence of a physical exam. -One staff file was not available on site for review.
The program's attendance records indicate 36 school aged children are operating as one group. The maximum group size for school age is 26 children.
