At the time of the visit, in a review of two students IHCPs and the corresponding medication consent documentation, the documentation was not fully complete. Student A was missing documentation of if they had previously taken the medication (on one, out of two) of their medication consent documents.
Current consent for administered medications
At the time of the visit, in a review of the emergency drill log, documentation of the March 2025 drill was missing. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
At the time of the visit, in a review of four staff files, the documentation was not fully complete. Two staff were missing documentation of bi-monthly observations and one educator had expired bi-monthly observations. Educator C was missing documentation of an annual evaluation. The program must maintain personnel records for each staff member as outlined by the staff record checklist.

Individual Health Care Plan (IHCP) - R Apr 2025
At the time of the visit, in a review of three staff files, two were missing documentation of staff observations. The program must maintain a personnel record for each staff member.
At the time of the visit, in a review of two student's IHCPs the documentation was not fully complete. One student had a doctor provided plan (REDACTED) but was missing documentation on the diagnosed condition, educators trained on the condition and consequences if medication was not administered. The same child had an IHCP for (REDACTED) that was missing documentation on consequences if medication is not provided and educators trained on the condition. Another child had a doctor provided IHCP for (REDACTED) but their EEC IHCP stated the child had (REDACTED). This child's doctor provided IHCP stated that the child (REDACTED), however the program did not have a copy of the child's (REDACTED). At the time of the visit, in a review of the medication consent documentation pertaining to the two students IHCPs, the information was not fully complete. Please ensure all aspects of the medication consent document is completed. Once student had (REDACTED) on site, but did not have a medication consent form.

The building and fire inspection were expired as of December 2022. The program has been in contact with to schedule an inspection.
A review of three medication consent forms indicated that one did not have evidence of a current parental consent form.
A review of three Individual Health Care Plans indicated that all three did not indicate which staff were designated to administer the medication in case of an emergency.

It was observed in the PreK classroom that emergency medication was not readily available while the children were outside on the playground. This indicated that the child did not have what was needed in case of a potential emergency.
The following were observed and accessible to children: INFANT/TODDLER - Near the secondary exit door, the white play kitchen was not secured and tippable. TODDLERS - In the Dramatic Play area, there were two plastic bassinets and high chair not secured and tippable.
The following were observed and accessible to children: SCHOOL AGE - In two unlocked counter drawers, there were two electrical cords approximately five feet long; potential cause for a choking hazard.
Staff Schedule/Qualifications - R Sep 2025
The following was observed and accessible to children: PREK - In an unlocked "Teacher Cabinet", there was a staff purse. SCHOOL AGE - In an unlocked counter cabinet, there were plastic bags. KITCHEN (barrier open) - In the unlocked cabinet under the sink, there were bleach bottles with manufacturer's directions to "keep out of reach of children". In a drawer with a broken lock, there were approximately five sharp knives.
A review of the Infant 1 and 2 attendance record indicated that three out of seven children were not signed in.
Equipment is clean, safe, secured, and poses no hazard.
