During the visit, one class was observed prior to eating. Children were playing with toys and books at the table. Children were seated at the table. Children did not wash their hands, and the table was not cleaned, prior to eating.
The following were observed outdoors: large chip out of playhouse base (creating sharp edge), The following were observed indoors: -Rags used for cleaning left accessible to children in two rooms. -There was exposed plaster in the Unicorn bathroom. -Several vinyl chairs/ equipment were torn or frayed, exposing cloth/ padded interior that can not be washed/ disinfected. -Gloves were accessible to children in one room.

During the visit, the Little Bears room had 5 children present but only 4 children signed on the attendance record.
During the visit, a high chair table, with food remnants on it was on top of the diaper changing table. A common changing table or diapering surface is not used for any other purpose.
In the Disney babies room, one child's hands were not washed after diapering.
In the Disney babies room, it was observed that the diaper changing surface was not covered by a disposable cover while one child was diapered.
