Individual Health Care Plan (IHCP) - R Apr 2025
In the infant room, two cords and an outlet were accessible through the slats of the crib.
During the visit, several preschool children were observed laying on the floor with a small carpet square. The square was about 12 inches. These children were not provided an individual cot for rest time.
Windows on the second story were not adequately modified to prevent opening more than six inches.
