The following were observed outdoors: -The pea stone was not an adequate depth around the climbing structure. There were protruding bolts. -The sandbox was within the use zone for the monkey bars.
Indoor Space
During the visit, a candle with an actual flame was burning. Staff extinguished it during the visit.

Staff meetings
One grate on the foundation had chipped paint and rust.
The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. One plan reviewed did not describe its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
Although the director indicated that meetings are held regularly with staff, the program did not have documentation of meetings every month. In addition, the documentation did not always indicate the length of the meeting.
