At the time of the visit, in a review of four educators files, the information was not fully complete. One educator had an expired PQR. The program must maintain a personnel record for each staff member as outlined by the staff record checklist.

Bottles/pacifiers/toothbrushes/sleeping materials labeled with child's name
At the time of the visit, the current EEC License was not posted within the program. The licensee must post the following information in an area easily visible to parents, educators and visitors: the current license or approval.
At the time of the visit, in a review of the first aid kits on site, it was observed that the kits were not fully complete. Both kits were missing a gauze roller. The licensee must maintain adequate first aid supplies, including, but not limited to: adhesive tape, band aids, gauze pads, gauze roller bandage, disposable non-latex gloves, instant cold pack, scissors, tweezers, thermometer, and CPR mouth guard.
At the time of the visit, it was observed that two students bottles and one students pacifier were not labeled with their name. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, and sleeping materials, must be labeled with the name of the child for whom they are intended.
At the time of the visit, it was observed that a play kitchen within the Toddler Classroom was not secured and was tippable. The licensee must ensure indoor and outdoor equipment is sturdy, safely constructed and installed and non-tippable.
At the time of the visit, in a review of four educators files, the information was not fully complete. Two educators were missing documentation of their physical.

At the time of the visit, in a review of three students files, one students file was not fully complete. The student was missing Developmental Placement documentation. Whenever a child is considered for assignment in a fixed age group outside their chronologically defined age group, the licensee must document all required information and maintain the information within the students file.
