Diapering mat shows wear and tear and needs to be replaced.
Two files were reviewed. One file needs an updated physical, lead screening, and medical documentation that there is no longer an (REDACTED) is not needed.

Educator has one cat. rabies record could not be reviewed during this visit. Educator will need to submit copy of cat's rabies record.
There were four files that did not have current progress reports on file.
Annual Registration in Professional Qualifications Registry (PQR)
Attendance records were reviewed. It was observed that there was a child signed in and signed out for today’s visit, although child was still in care. Educator must sign children in and out to reflect actual time. Attendance records for assistant is not being documented and educator is not signing in and out when she leaves the program.
Emergency preparedness plan could not be reviewed educator did not have a hard copy of her parent handbook. A copy of emergency preparedness plan will be required to be submitted.
Evacuation logs could not be reviewed; educator did not have them accessible. Evacuation drill was observed during this visit.
First aid kit did not have emergency cards, tape, gauze pads, disposable non-latex gloves, gauze roller bandage, and CPR mouth guard.
Ongoing professional development trainings could not be verified. Educator will need to submit and register her ongoing training into her PQ registry to be verified.
Educator has a personalized parent handbook, however does not have required EEC information. Discussed with assistant being able to give a copy of EEC’s parent handbook and educator’s personalized parent handbook to the parents.
Assistant file could not be reviewed. Educator must keep a file for her assistant with all ongoing professional development trainings and current physical.
The licensor observed an infant napping on the floor on top of a blanket. The infant was immediately moved to a pack and play. Parent safe sleep letter will be emailed to the educator, as well as the safe sleep pledge for the educator to sign. Educator will be required to retake the safe sleep training. A follow up visit will be completed.
