Individual Health Care Plan (IHCP)
The wall between the Toddler/PS and Toddler/PS 2 rooms and was moved and the program did not notify EEC. The notification to the Department at least 30 days prior to any change in the space used by the program required by 606 CMR 7.04(15)(f), must be made in writing. If the licensee is unable to provide 30 days’ notice, the licensee must notify the Department by telephone immediately upon learning of the impending change and must send a written report to the Department within 48 hours of notification.
The following was observed to be accessible to children during the visit: In TODDLER/PRESCHOOL 2, a long speaker cord hung loosely between the clock/speaker and the outlet.
The following was observed in the outdoor space during the visit: In the Toddler area, the basketball hoop and kitchen set were not secure and tippable and all along the back of the fence between the program's chain link fence and the taller vinyl fence is unknown vegetation growing.
Two children's IHCPs were incomplete. They were both missing the names of educators trained to administer medication and who trained them.
One child's (REDACTED) was not available during the visit.

Educators First Aid and CPR Certified
The educator that administered first aid to a child (REDACTED) was not currently certified in First Aid. Their certification expired 3/15/2024.

The following was observed to be accessible to children: -In Preschool 1, the cord for the boombox needs to be secure, the rug near the cubbies is unraveling and the strings are loose, the corner at the far end of the wall near the office is cracking and plaster has fallen off, four ceiling tiles are stained, baggies of wipes and one container of skin ointment were on open shelves under the changing table and have a label to "Keep out of reach of children", and the wooden kitchen set has a broken piece on the top shelf. -In both of the Preschool 2 bathrooms, the floor boards behind the toilets have peeled off and exposed loose and cracking dry wall; In Preschool 2, the wooden kitchen set has a broken piece on the top shelf and one ceiling tile is stained; In PreK 1, the bathroom right hand door under the sink is peeling; In Toddler 2 the half wall behind the irregularly shaped table for 4 is cracked; and most of the bathrooms have holes and some peeling paint on the walls where the old toilet paper holders were hung on the walls.
The directors stated they were not aware infants, toddlers and children with identified special needs must have progress reports completed every three months. The regulation addressing progress reports was emailed to the directors on 11/15.
This investigation inspection recorded no violations or advisories.
The following was observed to be accessible to children: -In PRESCHOOL 1, the toy tool bench was not secure and tippable. -In PREK 1, the refrigerator is sitting on top of a shelf and not secure. It is tippable. -In PREK 2, an open box of gloves was found sitting on the half wall behind the water station. The box was moved during the visit. A can of shaving cream was also found in the unlocked cabinet near the bathroom. The can was moved to a higher shelf during the visit. -In the PREK 2 BATHROOM, a plunger was sitting next to the toilet and the wall was peeling under the toilet paper holder.
A review of three staff records was conducted and found: -One staff record was missing evidence of completion of EEC Essentials trainings and MMR. -Two staff records were missing current CPR/First Aid certification (both expired 10/2021). -All three staff records were missing bi-monthly observations and annual evaluations. Each was observed in October 2023 but no documentation of observations prior to then was found. Two staff began working in 2020 and the third began working in 2018.

On the Playground the following hazards were observed: containers, in which the shade posts are connected to, have sharp unprotected edges. A 3 foot plastic slide is not secured and is tippable. Two awning poles are not covered with an impact absorbing material; these posts are metal. A structure with swings (that the director stated was put up this past weekend) had two swings that do not meet EEC guidelines. A Playground Safety from Consumer Products Safety Commission has been sent to the program via email.

In Preschool 2 a small carpet in the reading area was turned up at the corners, resulting in a trip hazard. In PreK 1 one wooden shelf was not secured to the wall.

The diaper trash in the Preschool 1 classroom next to the changing table did not have a cover.
The evacuation procedures were missing from the doorway of the Preschool 2 room that entered the Preschool 1 room.
Two outdoor play structures and one dome climber were observed to have been installed however they did not have sufficient amount of use zones (must be at least six feet in each direction). The Public Playground Safety Handbook was emailed to the directors.
The Staff Meeting Binder was reviewed and found there was only evidence of meetings between March and November 2024 and the following information was missing: attendance of staff and duration of time of the meetings.

Three staff records were reviewed and the following was observed: -Two staff records have incomplete staff orientation checklists (missing the dates of completion, number of hours, and name of the person who conducted the orientation). -Two staff records do not have evidence of completed EEC Essentials 2.0 (hired in September and October 2024). -One staff record was missing a physical and the second date for MMR vaccination. -One staff record is missing annual evaluations and a bi-monthly observation from September 2024.
One outlet cover was missing from the outlet in the Preschool 1 bathroom.
The names of two children and their medical needs were posted in one classroom.
The following was observed: Scissors are needing in the Toddler/Preschool First Aid Kit and a CPR mouth guard is needed in the Preschool 1 First Aid Kit.
In Preschool 1, an Elmo doll sat on top of the changing table. Some of the children in this group are still wearing diapers.
The changing table pad in the Toddler/Preschool classroom had a large tear at the top left corner.
An antihistamine for one child and an Albuterol Inhaler for another child were not available during the time of the visit.
The program provides morning and afternoon snack, however, they do not have a menu.

The Building Inspection expired 11/8/2023 and the Fire Inspection expired 3/21/2023.
The following was found to be accessible to children: -A toy tool bench and free standing basketball hoop were not secure and tippable.
A review of all emergency medications was conducted and found: One child's EpiPen was expired (9/2023).
