Two staff records were reviewed during the visit and found one record did not have a Staff Information Form and the other staff record did not have evidence of a second reference check.
The following was observed to be accessible to children during the visit: In OSPREYS, one spot of paint had peeled from the middle of the right hand wall where a hook had been hanging; in CHICKADEES, the baseboard along the wall with the windows in the dramatic play area had a spot where paint chipped off; and in SPARROWS, the kitchen set was not secure and tippable.

The following was observed to be accessible to children: there is a large whole in the fencing between the first and second space- this is dangerous a child could stick their head or appendage through and get caught and three plastic baskets hanging on a board are cracked and broken.
Staff Schedule/Qualifications - R Sep 2025
Hammocks were observed in each outdoor space and the ground surface underneath and use zone were not suitable. The use zones under and around swings, slides, and climbing structures must be covered with an adequate depth of an impact absorbing material, in accordance with EEC policy. Pea gravel and wood chip nuggets must not be used in areas used by infants and toddlers.

A review of all Individual Health Care Plans was conducted and found: -One child's IHCP was not complete. It was missing the name of the educators trained to administer the medication and who trained them.
Two staff records were reviewed and found: One staff record was missing bi-monthly observations since they were hired in November 2023. The licensee must maintain in each personnel record: documentation of ongoing supervision and annual evaluations.

The following was found to be accessible to children: -In the bathroom, a plunger sat next to one of the toilets.
This monitoring visit inspection recorded no violations or advisories.
One shelf unit in the PreK Room had loose screws, coming out and not secured.
Outside of the playground door (from Studio Room) is an HVAC unit, with sharp edges accessible to children

A review of the program's emergency evacuation drill log indicated that no evacuation drill was conducted in January 2022.
A staff purse was accessible to children in an unlocked closet in PreK; this was moved at the time of the visit. A roll of plastic bags was accessible to children in an unlocked cabinet in Preschool Room; this was moved at the time of the visit.

The current staff schedule was not posted for in an area easily visible to educators, parents and visitors.
The following was observed to be accessible to children: In OSPREY, the cord to the air purifier was long and loose on the floor; and in PRESCHOOL, a large bottle of foaming hand soap was under the sink (unlocked) and has the label "Keep out of reach of children".
Three children's IHCPs were incomplete. Two plans did not have potential side effects listed. One plan did not have the name of the educators trained to administer the medication.
One child has (REDACTED) in Toddler Room; however, this is not posted.
Two Individual Health Care Plans (one for an (REDACTED), one for an(REDACTED)) were not signed and dated by the parent/guardian.
