A review of the staff meeting binder was conducted and found the times and duration of the meetings are missing and the date for the March 2026 meeting was missing.
The following was observed during the visit to be accessible to children: In TODDLER 1, the green and white bookshelf in the middle of the classroom is chipped on the outside and the particle board is showing and the wall separating the classroom from the bathroom area is also chipping at the bottom near the bookshelf; in the TODDLER C/D and PRESCHOOL B bathrooms, the wall water spouts are uncovered and is hazard to children if they were to fall on it; in TODDLER 2, the bottoms of the cabinets near the dishwasher and refrigerator are peeling; in PRESCHOOL B, the wall to the left of the mirror has a lot of sticky tape residue, three spots on the floor are stained dark brown and the shelves and corners of the room have visibly crumbs and dust; in the PRESCHOOL D bathroom a plunger was sitting next to the toilet and multiple baggies containing wipes were found in the unlocked drawers (have the warning to Keep out of reach of children); and in PRESCHOOL A one of the drawers contained a pair of adult scissors.

The following was observed during the visit to be accessible to children: Outside in the Preschool area, the baseboard along the bottom of the building has peeling paint (3 areas).
Two children's IHCPs were not complete. One was missing the potential consequences. Another child's IHCP was missing the names of the teachers trained to administer the medication. It stated "All faculty".

Three staff records were reviewed and found: One staff record did not complete the required 20 hours or professional development (they had 17.5 hours) and they had bi-monthly observation missing in April and August 2024; and another staff record was missing bi-monthly observations in March and August 2024.
The attendance in PRESCHOOL 2 was not accurate. It documented that ten children were in attendance but there were twelve children. This was fixed during the visit.

The following was found to be accessible to children: -In PRESCHOOL 1/MULTIAGE B, the wall behind the toy kitchen set was covered with sticky residue that cannot be cleaned. -In the PRESCHOOL 2/MULTIAGE D bathroom, a plunger was sitting next to the toilet.
The Building Inspection is current and expires on 9/1/2024. The Fire Inspection expired 3/2/2024 but the Director stated once two emergency lights are updated by an electrician, the fire department will issue the new certificate.

This investigation inspection recorded no violations or advisories.
The following was observed to be accessible to children: -Rust is visible and chipping at the bottom of the door frames and doors on the classroom doors leading to the outside in TODDLER B, TODDLER D, and MULTIAGE B. -The half door in TODDLER A leading to the adjacent toddler room was blocked by sleep mats and caused an obstruction to the egress leading directly outside. -The bathroom in between TODDLER A and the adjacent toddler room (closed today) contained two brooms leaning against the wall, and three cabinets unlocked which contained wipe containers, a box of opened gloves, and loose garbage bags. Licensor used the locks already on the cabinets to lock them. -The long bookshelf closest to the bathroom in classroom TODDLER D has begun to chip on top in the middle, exposing the particle board. -In MULTIAGE B, the cabinet to the right of the dishwasher contained a loose plastic wrapper (thrown away during the visit) and the two cabinets to the right of the dishwasher contained stains and crumbs on the bottom of the insides. -In the bathroom in MULTIAGE B, a plunger was sitting to the right of the toilet and a drawer contained 2 rolls of loose bags, a box of opened gloves, and a bag of wipes. -In MULTIAGE D, one of the white boards on the opposite wall of the bathroom contained many crayon wrappers. -In the MULTIAGE D bathroom, a drawer contained a box of opened gloves, a bag of wipes, and Aquaphor with the label “Keep out of reach of children”.
The following refrigerators were found to be over the required 40 degrees: -The refrigerator in INFANT B was approximately 42 degrees. -The refrigerator in MULTIAGE D was approximately 42 degrees.
The program does not have a current Fire Inspection. The previous once expired on 1/14/2023.
The posting for the location of the first aid kit in the PREKINDERGARTEN classroom was not accurate. The posting indicated the First Aid Kit was on a shelf on the left hand side of the classroom and the First Aid Kit was observed to be hanging on the outside exit door at the bag of the classroom.

Observations from original visit on 2022-08-18 00:00:00: Staff in INFANT 1 and TODDLER A stated separate sinks for handwashing and food preparation are not being utilized. Corrective Plan received on 2022-09-08 00:00:00: Both sinks have been cleared of toys/warming cups. All staff have been retrained on proper bottle warming and proper placement of mouthed toys (Mouthed toy bucket). All staff have been retrained on using the correct sink for handwashing and the correct sink for food prep. Observations on 11/1/2022 in INFANT A: -A spoon was found in the handwashing sink indicating the sinks are not used for separate purposes. Observations on 11/1/2022 in INFANT B: -A pacifier and dish was found in the handwashing sink indicating the sinks are not used for separate purposes.

The following were observed in the indoor space: a metal rack unanchored in the infant hallway; tape residue on the wall with windows in INFANT 1; area rugs with visible dirt and crumbs, broken foam bumper on the window and tape residue on the empty shelf next to the door in TODDLER A; peeling wood at the children's sink, an accessible dust pan, brush, and broom, sticky spots on the floor, cabinet doors near the sink covered with dust and the inside shelving gathering dust, a loose plastic bag under under the cabinet, area rugs all contained crumbs and loose dirt, tape residue on the walls in the library area and on the wall to the right of the exit door, and trash can cover covered in old food and spots in PRESCHOOL 1; seven chairs stacked and unsecured, tape residue on the walls, open windows with broken screens, six tall, loose screens leaning unsecured against the wall , a box of trash can liners in the cabinet near the sink, a loose extension cord, yarn, and plastic bag on top of the PPE boxes in the BOARD ROOM; toilet paper not sanitary in the bathroom, area rug covered in crumbs and mulch pieces, tape residue on walls in the kitchen area; and a loose white string of yarn near the side door in PRESCHOOL 2; piled equipment, unanchored shelving, and tripping hazards in an empty KINDERGATEN classroom accessible to Preschool. Cobwebs and dusty ceiling air vents were also seen throughout the program in CLASSROOMS and HALLWAYS.

The following was observed to be accessible to children: In the PRESCHOOL 1 bathroom, a box of open gloves was found in the middle drawer; in PRESCHOOL 2, two adult scissors were found in a classroom drawer, the white and green bookshelf had a broken divider and holes revealing particle board, an open box of gloves was in the bathroom drawer and a toilet brush was on an open shelf in the bathroom; and in TODDLER A, the left hand bracket on the wooden stove was sharp at the corners.
Two staff records were observed during this visit and found: -One staff record did not have evidence that the staff was vaccinated against MMR. -One staff record did not have a complete Staff Information Form.

All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging.
The following was observed to be accessible to children: -Two of the white doors on the outside of the program in the Preschool Playground had large areas of peeling paint. Pieces of the paint were visible on the ground.

The attendance in INFANT A was observed to be inaccurate. Six children were marked as arrived but seven children were in attendance.

The following was observed in INFANT A: -A child's prescription REDACTED) was not in the original container in which it was originally dispensed and with the original label affixed.
Observations from original visit on 2022-08-18 00:00:00: Emergency medications for children in the INFANTS 1 class were not readily available as required. They were locked in a classroom closet. The first aid bag containing first aid materials and emergency medications for TODDLER A was not readily available as required. The bag was left outside when the group returned to their classroom. A review of a child's Individual Health Care Plan in PRESCHOOL 2 indicated the child needs two additional medications (Flovent and Singular). These medications were not provided by the parents and not accessible to staff. Corrective Plan received on 2022-09-08 00:00:00: -The medication from Infant 1 was placed inside the emergency bag. - All staff have been retrained on the emergency bags and keeping them with them at all times. -The preschool 2 child with the two additional medications that are preventative medications are only to be given at home in the morning and evening no longer attends our program. Observations on 11/1/2022: A review of a child's Individual Health Care Plan in TODDLER A indicated the child needs one additional medications (REDACTED). This medication were not provided by the parents and not accessible to staff. The Educator in INFANT A stated staff lock the closet door located in INFANT B which is the location for the emergency medications for INFANT A.

Last physical done on 6/15/2021. -One child's Individual Health Care Plan was incomplete (missing symptoms, side effects of medication, and consequences). -One child's transportation plan expired. -One child's Developmental History expired 8/1/2022. Corrective Plan received on 2022-09-06 00:00:00: -Enrollment Form has been completed - Physical has been updated. -Transportation plan and developmental history- the child no longer attends went to public school. Observations on 11/1/2022: The licensing staff reviewed two children's records available at the program noting: -One child's medication consent forms (2) expired on 10/28/2022. The licensing staff also reviewed all Individual Health Care Plans for children with allergies and/or chronic illness and observed four children's plans were not signed and dated by the parent.

Visit of 11/1/2022 The following was observed to be accessible to children in the Women's Bathroom: -Two toilet brushes -Two plungers The following was observed to be accessible to children in the Men's Bathroom: -Plastic bags -A plunger -A toilet brush The following was observed to be accessible to children in the INFANT HALLWAY: - Three trippable floor mats The following was observed to be accessible to children in TODDLER A: -Ripped foam padding along the windowsill -Carpet trim missing and collecting crumbs as well as starting to unravel. -A hole in the screen on the window on the right hand side. -The window screen on the left hand side window did not fit properly as evidence by a space at the top right corner. -Tape residue along the left side wall. The following was observed to be accessible to children in the TODDLER C: -Tape residue along the inside of the water table. -Sink in dramatic play area contained dust. -A trippable rug under the sink in front of the children's bathroom sink. -A toilet brush and broom next to the children's toilet in the bathroom. -An uncovered hose spigot next to the children's toilet appeared to be sharp. The following was observed to be accessible to children in the TODDLER C: -Floor between the sink and the refrigerator appeared brown and contained crumbs and dust. -Tape residue and peeling off the sink in the dramatic play area. The following was observed to be accessible to children in the PRESCHOOL 1: -A broom and dustpan in the children's bathroom. -A cabinet containing five jars and tubes of diaper creams. -Tape residue on the bookshelf holding puzzles and blocks and on the wall to the right of the outside door. -Labels peeling off the shelves and leaving stickiness behind no the bookshelf to the left of the outside door. -Lego table and sink in the dramatic play area contained dust and crumbs. -A pair of adult scissors on the counter above the refrigerator. This was moved during the visit to be inaccessible to children. The following was observed to be accessible to children in the PREKINDERGARTEN: -The floor next to the first trashcan and between the tables appeared to be sticky and dirty. -The top of the bookshelf adjacent to the Lego table peeling off at the edge and exposing the cork wood material. -Tape residue on top of the Lego table, the table in the middle of the room, the bookshelf in the dramatic kitchen area, and on the walls in the kitchen area as well as in the calendar area and the cabinets next to the sink. -An adult purse hanging low in the unlocked closet. -A broom leaning against the wall next to the sink. -One trash can uncovered and the cover of the second trash can covered in spots of stickiness and crumbs. -Two trays attached to the magnetic letter boards contained dust and crumbs. -Two toilet paper dispensers broken and missing pieces in the children's bathroom. -A used dust pan laying on the floor to the left of the sink in the children's bathroom. -Two drawers unlocked in the children's bathroom contained loose plastic bags. The following was observed to be accessible to children in the Hallway of the PREKINDERGARTEN area: -A rusty copper pipe. This was removed during the visit. -The white vent at the bottom of the wall to the left of the outside door not secured and exposing the hole in the wall.
Observations from original visit on 2022-08-18 00:00:00: Small holes in the walls near the bookshelf in TODDLER A were accessible to children. Corrective Plan received on 2022-09-06 00:00:00: Placed work order to have the holes patched. On 11/1/2022, paint was observed to be chipped all along the window sill in the PREKINDERGARTEN classroom.
When reviewing emergency life saving medications in classroom PREKINDERGARTEN, it was observed a child had been given (REDACTED) on 4/15/2022 as a result of a possible allergy exposure when he ate a bite of another child's donut. The Licensee and Assistant Director stated they do not have documentation of parent notification of emergency treatment as required by 606 CMR 7.08(8)(e).

A review of the documentation of monthly staff meetings indicated meetings have not been conducted with all staff monthly as required. - The program did not have documentation that meetings were held in July 2021, September 2021, December 2021, May 2022, June 2022, and July 2022.
