In the nap-room it was observed there were storage bins stacked on top of each other causing a tipping hazard. It was observed that there was access to plastic bags and cleaning supplies under the bathroom sink. There was access to diapering wipes under the changing table in the bathroom.

Toothbrushes were labeled with names but not with date toothbrush was received. This will help provider know when toothbrushes need to be replaced.
There was not an appropriate barrier installed around the wood stove to prevent access. Discussed requirements for barrier being 3 feet out and 3 feet tall of a material that will not retain heat.

The educator’s PQ number expired on 4/2015. The educator is having issues login to her PQ registry. Licensor has submitted a ticket and waiting to get a response.
Educator did not have her evacuation log on hand.
Child’s (REDACTED) was expired and (REDACTED) did not have child’s prescription information with it.
There is access to the laundry room from the napping room, which it was observed there was access to cleaning products such as, laundry detergent, bleach, and other products. Room temperature in rooms occupied by children must be maintained at a minimum of 65 F. It was observed that the rooms felt a bit cold, when temperature was checked it was reading between 61-62 degrees.

Emergency list numbers did not include educator’s address and telephone number and emergency backup person information was not clear.
There is one child enrolled with a medication allergy this was not posted as required.
The educator’s attendance records mostly reflect actual arrival and departure times. It was observed that there were days in which children were not signed in and/or out. It is observed that the educator writes in the name of the full time children on the first day of each week and just writes the times of arrival and departure across for the rest of days in attendance. When other children attend throughout the week she writes in their names. It was discussed with the educator that in order to have accurate records she should write in the children’s names or initials for each day.
