At the time of the visit staff meeting documentation was not available.
At the time of the visit the mulch was not 9 inches at all areas where a child could fall from the structure.

During a review of 2 chronic medical conditions it was learned that the program did not have all EEC required information in the plans, including "who trained' the educators in the child's medical condition and "who was trained" in the child's medical condition.
