Supervision - Napping Children
Child E - 1) The allergy plan was not dated, missing the potential side effects of medications and potential consequences to the child if the treatment is not administered. 2) The program did not have one of the emergency medications listed on the plan available to administer if needed. 3) The Certified Assistant was not aware of the full list of food allergies listed on the record and posted in the program. The Certified Assistant reported to the Licensor that the child was allergic to one food when the records list four. Child F - 1) Missing Individual Health Care Plan for food allergies listed on medical form 2) The Certified Assistant did not mention this child's name when asked if any children had chronic medical conditions in the program. Child G - Missing Individual Health Care Plan for a food allergy listed. The Certified Assistant in charge of the program was not prepared to implement the individual health care plans for the children in her care.

Bathroom: hair products, ointments, razor, shaving cream, lotions, toothpaste accessible in unlocked drawers and on top of the vanity. Kitchen: zip lock bags, aluminum foil with serrated edges, medication, cleaning products in unlocked drawers and cabinet.

1) The Certified Assistant's infant was not listed on the attendance on October 28th, and 29th, 2025. 2) Missing assistant's attendance.
The Certified Assistant did not exercise appropriate supervision of the children in her care in order to ensure their health and safety at all times. There was inappropriate supervision of the napping children.
The licensee did not maintain sufficient numbers of qualified staff to promote the health, safety, growth and development of each child when she left the Certified Assistant caring for four children under two years old, including three infants by herself.
Six children present at the time of the visit. Four children under two years old including three infants. The Certified Assistant was working alone. The educator was not present. This is the second non-compliance with the program operating in excess of infants.
There was inappropriate supervision of the three napping children. The Certified Assistant did not keep the door ajar for proper supervision when she placed the children to nap in the room with the door closed. The Certified Assistant admitted that she was relying on a baby monitor for supervision.
The Certified Assistant did not use good judgement when determining the appropriate level of supervision of three children in care on the day of the visit.
The Certified Assistant was not positioned to maximize her ability to see and/or hear three children in her care. The children were sleeping behind a closed door in the nap room.

Child E - 1) parental permission for medication administration for one emergency medication is expired. 2) Missing parental permission for medication administration for two additional emergency medications.
Kitchen: knives and plastic bags in two drawers with locks not engaging. Bathroom: cleaning products, toiletries, air freshener in the cabinet under the sink. The lock was not engaging.
Three children were napping in an unlicensed and unapproved room at the time of the visit.
The infant was napping in the unlicensed room with a bib around his neck. The sleeping equipment has been altered with a mattress on top of the pack and play's original mattress. There was a gap of about two inches in between the mattress and the wall of the pack and play. There was inappropriate supervision of the napping baby, the door was closed and the Certified Assistant was relying on a baby monitor for supervision.

Bathroom: three drawers with toiletries, razor, toothpaste with locks that were not engaging. Please replace the locks.
