Staff pocketbooks were observed accessible to children in the large room. Cords to computer, fan, string lights were observed hanging accessible to children. Surge protector was on the floor in the large room and were missing outlet covers in the unused plugs.
IHCP on file one child's was missing a signature and date. One IHCP was expired.
Written emergency evacuation procedures were not posted at each exit.

6 Children's files were reviewed. 2 children who were present at the time of the visit did not have a file onsite. 3 Files had all expired consent forms, physicals, developmental history, no evidence of progress report and offsite consent forms. 1 file was missing current physical, immunizations, and lead screening.
Staff records reviewed were incomplete. They did not include documentation of references, immunizations, current physical, current PQ registry, staff orientation, staff information form, ongoing bimonthly observations, Annual evaluation, and evidence of staff training.
At the time of the visit the pre-k classroom had 10 children, but attendance only had 9 children marked in.
At the time of the visit there were shelves in the preschool and pre k classroom that were tippable and not secure. At the time of the visit there were bookshelves that were tippable and not secure. A shelf in the large room was tippable and not secure. Two cubbies in the large room were tippable and not secure.
Evacuation Drills (Fire Drills) were not documented as required by regualtions.
One child who had an ongoing medical condition did not have dissolvable [REDACTED] per his IHCP.
Educator did not wash and disinfect the diaper changing surface after each diaper change.
Table in the toddler room was only washed not disinfected prior to having snack.
Current consent was not on file for medication on site at the program.

A review of staff files found the following: Two staff did not have a completed Staff Information Form One staff did not have current medical information on file. One staff did not have evidence of completed EEC essentials. Some staff were missing evidence of reference checks in their files. Some staff were missing evidence of Bi-monthly observations. Some staff were missing evidence of ongoing staff training.

Building and Fire Inspection were not posted so it could not be determined if it was current. The program was caring for children under the age of 2 yr 9 months and did not have the proper building code on the building inspection.

Open snacks were observed not in a clean covered container.

Children were observed going to the bathroom unsupervised. Staff in two preschool classrooms left children in the classrooms while they went out of the classroom. The Director left children in the classroom while she opened the door for the Licensor. Staff in the older preschool classroom left children in the classroom while she checked on several children who had gone out of the classroom to the bathroom.

During a review of 2 randomly selected staff records, the following was learned: One staff did not have evidence of references or a yearly staff evaluation as required. One staff did not have evidence of a staff form, a physical within the last year (date of hire 3/16/22), [REDACTED]records or program orientation as required.

During a review of the indoor space, the following was noted: chipped white paint was observed in both classrooms; adult scissor were left in the reach of children in the "gross motor" space, a cubby unit outside of the classroom door was observed to be "tippable."

At the time of the visit there were open snacks not in a covered container in several classrooms.
A potty seat was observed on the floor in the bathroom. Paper towels were observed on the floor in the bathroom.
At the time of the visit there was an expired [REDACTED]for one child who was not at the program for the summer. One [REDACTED]had an expired prescription, one child's [REDACTED]was missing a prescription label.
A review of IHCP found that not all children with ongoing medical conditions had all of the required information on file at the program.
Staff was observed changing a diaper and did not put a barrier between the child and the diapering surface.
At the time of the visit the program was using the nearby park. Children were observed using swings which did not have the proper depth of impact absorbing material.

A review of staff files found that not all staff had evidence of staff orientation in their files. A review of staff meeting documentation found that staff meetings were not held two hours every month. A review of staff files found that only the Center Director had documentation of required staff training.
At the time of the visit shelves throughout were tippable and not secure. At the time of the visit some of the heating registers in the classrooms were in need of repair. There was a cord hanging from the radio accessible to the children in one of the classrooms.
A review of Children's Records found that not all children had updated physical and lead screening.

A review of IHCP's found that not all children with ongoing chronic medical conditions had an IHCP on file.
A review of children's files found that medication consent forms were not current for Epi Pens and Albuterol.
At the time of the visit there was a paper cutter, adult scissors and staff bags accessible to the children in the large room.

At the time of the visit there were 20 children in attendance and the program is only Licensed for 18 children. At the time of the visit the program was caring for children under the age of 2 years 9 months which they are not licensed for.
Staff used hand sanitizer but did not wash her hands with soap and running water.
At the time of the visit a heater in the first preschool classroom was in disrepair. Holes in the floor in the large open area near the stage. Cords from lights in the large room were hanging accessible to children. Staff bags were observed in the large room accessible to the children. Plastic child gates were being used to block stairs but were not secured. Two vacuum's were observed in the large room accessible to children.
There was an expired [REDACTED] in the toddler/preschool classroom. There were two [REDACTED] with two different expiration dates. .Both were expired. The prescription on the [REDACTED] in the Toddler/Preschool room was expired.
Children and staff in the older preschool room were observed using hand sanitizer prior to snack instead of washing hands with soap and water.

2 staff files were randomly selected for review and one staff did not have documented evidence of completing first aid within 6 months of their hire date as required.
Three children's records were randomly selected for review and the following was learned: one child did not have a current face sheet, first aid/emergency consent form, on site consent form, transportation form, developmental history form or a current physical on record as required. One child did not have evidence of a current physical on file.
Not all food was observed to be stored in sealed containers as required.
