Some of the emergency card pages for the children located in the first aid kit are outdated. These pages must be updated annually by the parents. Discussed the parents re-signing and re-dating the bottom of the pages if no information has changed.

Random review of five children's files. Educator uses the EEC enrollment packet and Annual Update form. Two children had enrollment packets last signed by the parent in 2021 and no current annual update form. One child had an Annual Update form signed in 2022 and the last physical on file was in 2022.
Educator has not been recording daily attendance. Attendance must include the daily arrival and departure times of each child as they occur. Discussed the educator using a notebook to track attendance.
Deck and the 3-season porch are not used with the children. Gate is missing from the bottom of the deck stairs to prevent children access.
Above ground pool is covered and not used with the children. Gate at the bottom of the deck stairs leading to the pool is not locked.
Educator's CPR and First Aid certificates were reportedly expired. Please complete a new course as soon as possible and provide certification.

Educator moved her child care space from the first level of the home to the basement without a 'change of space' transaction/visit. Please complete a change of space transaction as soon as possible to add basement space and remove first level space.
Educator has insufficient professional development training. She has 18 hours, therefore missing 12 additional hours. Please complete and send training certificates as confirmation. Also, ensure 10 hours are completed per year moving forward.

Licensor walked through outdoor play space and observed the following hazards: There was a fire pit which was uncovered. Please ensure all fire pits are covered when not in use. There was a trampoline that was not made properly inaccessible. Please ensure the trampoline is removed or made inaccessible to childcare children. The fence is missing a locked door and one post. Please make these corrections to fence and upload pictures once completed.
Educator did not have emergency numbers posted on the main level of the home at the time of this visit. Please ensure these numbers are posted and include 911 reminder, address & phone # for program, poison control, emergency back-up person.
The water temperature in the bathroom was 139 F degrees. The temperature should not exceed 120F degrees. Please make adjustments to water temperature.

Licensor walked through the licensed space and observed the following hazards: 1st FLOOR: **PLAYROOM** No hazards. **LIVING ROOM** No hazards. **DINING ROOM** There were two junk drawers that had items such as scissors, batteries loose and cables inside. Please make these items inaccessible to childcare children. **3 SEASON PORCH** No hazards. **KITCHEN** There was dog food and water accessible. Please ensure children do not have access to pet's food and water. There was one drawer which contained lighters and knives, another cabinet had loose sandwich bags and aluminum foil, and another under the sink cabinet had cleaning products. Please make all of these items inaccessible to childcare children. **BATHROOM** No hazards. **BASEMENT** Licensor did not assess as Educator would like to remove basement space from license as it is no longer being used for child care. Outdoors: Licensor could not assess outdoor space as it was covered in snow.
Random review of children’s records were conducted and all had missing or outdated progress reports. Please upload plan on how to get records up to date within one week. Please make note of the following: *For school age children, the progress report must be prepared at least annually, at the midpoint of the child’s program year. *For toddlers and preschoolers, the progress report must be prepared every six months. *For infants and children with identified special needs the progress report must be prepared every three months.
Licensor walked through basement space that Educator wishes to license and the following hazards were observed: *Bedroom: There were outlets that were not covered; please cover to avoid serious injuries'. There was medication accessible; please move to ensure children have no access. There is a small television that is not anchored; please anchor to avoid a tipping accident. Please confirm once all items are completed. *Play Room: No hazards *TV Area: No hazards Licensor could not assess outdoor play space as it was covered in thick snow. It was not safe for Licensor to walk through.
Educator did not have an appropriate emergency preparedness plan. Please complete this and ensure it is in parent handbook.

Educator did not have a parent handbook prepared. Please ensure a parent handbook is completed and given to each family at the time of enrollment.
Educator changed an infant's diaper and did not wash the child's hand afterwards. Please ensure proper hand washing procedures are done after diapering. Additionally, after diapering, the Educator used the kitchen sink to wash hands. Regulations state, "Facilities used for hand washing after diapering or toileting must be separate from facilities and areas used for food preparation and food service." Please ensure the bathroom sink is used for hand washing.
Licensor walked through indoor space and observed one hazard: In the dining room, there was an unlocked cabinet that contained open alcohol containers. Please make these bottles inaccessible to child care children and confirm once completed.
Educator used one diapering mat and did not have a disposable layer. Licensor discussed diapering policy. Please respond with what you will be using as a disposable layer.
