At the time of the visit the building inspection was expired.
At the time of the visit the gate to prevent the children from going up the fire escape stairs was not attached properly. The slats on the gate were more than 31/2 inches which are an entrapment hazard. The fence closest to the parking lot has a gap at the side that a child could get through.

Impact Surface
At the time of the visit there were swings that did not have the six-foot use zone to the sides of the swing structure. ASTM 1292 is needed for the impact surface under the swings. Please be advised that children cannot use the swings until the proper impact surface and use zones are in compliance.

A review of IHCP's found that not all IHCP's had all of the EEC required information. Some Allergy forms did not have a date on the parent signature.
At the time of the visit a prescription medication had an expired prescription label and stated to discard after 05/29/24.
At the time of the visit there was an area on the playground where part of the fire escape had an entrapment hazard. The front and rear gates of the preschool playground latches were small allowing children to push open the gate without it staying latched. Overgrown weeds were coming through the fence on both playgrounds.
This follow-up visit inspection recorded no violations or advisories.
Indoor Space
At the time of the visit there was a tv on the counter in the Preschool room that was not secure. At the time of the visit teacher scissors were observed accessible to the children.
