At the time of the visit the sandbox on the preschool playground was in need of more sand.
At the time of the visit there was an expired [REDACTED] 5/31/2025. Expired [REDACTED] also had an expired prescription 5/31/2025.
At the time of the visit two infants were given snack without their hands being washed.
There was an expired [REDACTED] in the toddler room for a child with a food allergy.

At the time of the visit there was a piece of housekeeping equipment that was not secure and had a screw sticking out.
At the time of the visit there were children's couches and chairs that were in disrepair. One needed the slipcover to be washed and the others were peeling and needed repair. There was a small climber in the toddler room that was peeling and needed repair.

At the time of the visit there was not 9 inches of mulch around one of the infant toddler structures on the playground. There was not 6 feet around all areas of the slide structure on the Infant Toddler Playground.
