The fire inspection was expired as of 8/18/2024.
During the visit, it was observed that there were janitorial cleaning products with warning labels to "keep out of reach of children" accessible to children on the side of the CAFETERIA.
During the visit, it was observed in the CAFETERIA that there were long electrical cords near the stage and hanging on the wall from a stop light. All were unsecured and accessible to children.

In the cafeteria, brooms and dustpans were observed in the children's activity space. The heating vent cover next to the stage was dusty and bent in several places.

A review of ten children’s records indicated: -One file did not include a parent signature for any authorizations in the enrollment packet.
A review of three educator files indicated: -Two educator’s did not have evidence of a current physical examination. -Three educator’s did not have staff observations, when asked the Site Coordinator stated they have not completed them. -Three educator’s did not have annual staff evaluations. -Three educator’s did not have evidence of an annual program orientation.

A Change of Administrative Authority transaction was not submitted in LEAD, notifying EEC of the new Site Coordinator. The licensee must provide prior notification, in writing, to the Department of any change of the person designated by the owner or the governing body as having responsibility for the administration of the program.
Practice fire drills have not been conducted monthly as required. The last practice evacuation drill was documented on 11/2/2022. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
Two children's Individual Health Care Plans (IHCP) did not include all of the required information. A copy of the IHCP should be kept with the child's emergency medication for reference.

On the day of the visit, there was no completed attendance available for review. The attendance was completed in the cafeteria after the children had already been picked up by the educator's in their classrooms. The licensee must maintain daily attendance records indicating each child’s attendance, including arrival and departure times.
During the visit, the building inspection certificate and fire inspection certificate were not available for review. Please submit.
The following was observed and accessible to children: -In the cafeteria, there was one uncovered garbage barrel filled with food waste. Garbage must be kept in lined and covered containers and all trash containers must be emptied at least daily.
A child enrolled in the program who required an (REDACTED) did not have one at the program.
A review of ten children's records indicated: -Four Individual Health Care Plans (IHCP) were incomplete. -Two children who had a chronic medical condition did not have an IHCP.
