A review of three individual health care plans indicated that two did not indicate which staff were designated to administer the medication during an emergency and one did not have evidence of a parental consent form.

The following was observed: In CLASSROOM 1, there was an electrical cord not secured and accessible to children. In CLASSROOM 2, the space was utilized as activity space instead of its approved emergency usage only.
The fire inspection was expired. An inspection was scheduled for next week.
Two large clear plastic bags filled with soil were located in the outside play area and accessible to the children.

When going outside to the play areas, the toddler/preschool and preschool teachers did not bring the daily attendance outside with them. Each classroom did not have their own attendance record showing who was present in the classroom that day. The Licensee must have a method of knowing exactly who is present on the premises at any given point in the day.
The Evacuation Drill Log did not include all of the required information. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
An educator in the Toddler/Preschool classroom did not wash the child's hands or their hands after changing the child's pull-up. After changing a child's diaper, the educator and child's hands must be washed with liquid soap and water, and dried with individual or disposable towels.
