Staff monitor environment for hazards-Hazardous items not accessible to children
It was observed in the GYM, there were two exposed pipes that were not hot to the touch and a broken electrical heater grill, a potential entrapment hazard.
It was observed that a current staff schedule was not posted.
A review of three Individual Health Care Plans indicated that one did not include the potential side effects of the medication and potential consequences of not receiving the medication.
A review of the evacuation drill log indicated that monthly drills were not consistently documented.

It was observed that towards the back of the cafeteria, there were two containers of disinfectant wipes and an open box of gloves accessible to children.
A review of three children's records indicated that they all did not have evidence of the child's transportation plan to and from the program.

Four staff records were reviewed and were found to be incomplete. Three did not have evidence of a staff information form, work experience verification form, current physical examination. Two did not have evidence of a completed background record check and program orientation completed.
A review of one individual health care plan was found to be incomplete. It did not indicate what the potential side effects or consequences were and which staff were designated to administer the medication.
This follow-up visit inspection recorded no violations or advisories.
A Change of Administrative Authority transaction was not submitted in LEAD, notifying EEC of the new Site Coordinator. The licensee must provide prior notification, in writing, to the Department of any change of the person designated by the owner or the governing body as having responsibility for the administration of the program.

The attendance was inaccurate. There were thirteen children signed in but only eleven children present. The Site Coordinator had signed two children into the program before they had arrived. The arrival time must be noted for each individual child as they arrive to the program.
A staff schedule was not posted in an area easily visible to educators, parents and visitors.
The Site Coordinator was unable to locate the Fire Drill log for EEC to review. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
A projected one-year operating budget that estimated income and expenses was not available at the program for review. Please submit.

Staff CPR Training - R Apr 2025
During the visit, three containers of Disinfectant Wipes were observed in reach of children in the cafeteria. The containers were labeled with "Keep out of reach of children".
A review of two staff records indicated that one staff had completed an online CPR certification course in February 2022 however, they did not complete an in-person skills test as required. Online CPR training may be taken as long as the participant is required as part of the online training to demonstrate and successfully complete age-appropriate CPR techniques to a certified instructor. EEC will not accept CPR certification training that does not include hands-on skill demonstration.
A review of two staff records indicated that one staff had completed an online First Aid certification course in February 2022, however, they did not complete an in-person skills test as required. EEC will not accept first aid certification training that does not include hands-on skill demonstration.

The documentation of staff meetings did not include an end time. Programs must document regular staff meetings of at least two hours per month to consult with educators.
During the visit, the building inspection certificate and fire inspection certificate were not available for review. Please submit.
