Health and Safety
A staff schedule was not posted. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.
The children's activity space near the teacher's cabinets were not free from hazards and clutter.
Practice fire drills have not been conducted monthly as required. The last practice evacuation drill was documented on 3/9/2022. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.

The attendance was inaccurate. There were 21 children signed into the program but only 17 children present outside. It was learned that the Site Coordinator had 4 children inside with them. However, the group leader did not know which children were inside with the Site Coordinator. Staff must have a way of knowing who is present on the premises at any given point in the day.
Staff did not provide the children with a variety of materials to use while outdoors. Staff did not offer the children opportunities to get involved in outdoor activities or games.
The children were observed using an outdoor space that has not been approved for use by EEC. The following hazards were observed in this outdoor space: three truck plows and a step ladder were accessible to the children. Unidentified vegetation was growing through cracks in pavement. The wood was splintering on the picnic table. There was debris throughout the area.
The location of the first aid kit was not posted.
A Change of Administrative Authority transaction was not submitted in LEAD, notifying EEC of the new Site Coordinator. The licensee must provide prior notification, in writing, to the Department of any change of the person designated by the owner or the governing body as having responsibility for the administration of the program.
The following hazards were observed indoors and accessible to the children: a ride on floor cleaning machine, floor drying fan, mop bucket, mops, brooms, vacuum, and two spray bottles of cleaning chemicals.
A review of five staff records indicated bi-monthly observations were not completed this school year.
On the day of the visit, the program staff did not record the arrival time of each child. This was corrected during the visit.
A review of the Children's Records Checklist indicated progress reports were not completed. For school age children, the progress report must be prepared at least annually, at the midpoint of the child’s program year.
