In the Ramsay Lodge, children's coats and backpacks were observed in a pile on the floor. The white laundry baskets for children to store their belongings were not used. The licensee must provide sufficient space, accessible to children, for each child to store clothing and other personal items in a safe, sanitary manner.

During the visit, children were observed exiting the contracted school bus. Once the children were off the bus, the driver and monitor did not physically walked the interior of the bus to make sure no child was left on the bus. The transportation log did not include 2 signatures verifying that they physically walked through and inspected all seat on the bus to confirm that no child was left behind.

In the Jenkins Lodge, a Group Leader working with the children was not sure if any of the children in the group had an allergy or used emergency medications. Educators must know if a child in their care has an allergy or life saving medication on the premises.

During the visit, children were observed exiting the contracted school bus and entering the licensed program. The following was identified: 1. The bus driver and/or monitor did not inspect the bus once the children had exited, to ensure no child was left on the bus. 2. All three Bus Logs were reviewed. The children from the HBB school were not signed off the school bus. The MECC and HBB attendance logs did not include a second signature confirming that the bus was inspected to ensure no child was left alone on the bus.

During the visit, children were observed walking to the play area while the gate to the black chain-link fence was left open. A section of the temporary silver fence was moved, leaving an opening in the fence. A group leader replaced the section back where it belonged. The Licensor asked another group leader to close the black chain-link gate. Staff must inspect the outdoor play space for hazards prior to the children using it.

During the visit, three staff files were reviewed. Two files did not include Verification of School Age Work Experience. One file did not include evidence of an orientation to the program.
During the visit, peeling paint was observed on the exterior of the Ramsay Lodge Building.
The barrier installed along a portion of the waterfront of Tispaquin Pond does not prevent the children enrolled in the program from reaching the water. The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children.
During the visit, it was identified that the transportation attendance log did not include 2 signatures, confirming that the vehicle was inspected after each trip and no child was left on the vehicle.

In the K-1 group, several children freely opened the exit door. Three children were observed opening the exit door and walked outside on the wooden ramp without a staff member's knowledge. Children opened the bathroom doors while other children were using the bathroom.
At the time of the visit, only the Arts & Craft/Science building was open. The K-1 group daily attendance was not available for review. The licensor was told the attendance was left in the front building. The 2-5 group daily attendance was not available for review. There were 17 children observed in the 2-5 group. However, the group leader stated there were 21 children present. There was no attendance form available to confirm how many children were signed into the group. The group leader recounted the children and confirmed to the licensor that there were 17 children present in the group. Children were not signed out of the group as they were dismissed. While outdoors, the staff in the K-1 group did not take attendance before transitioning the children back inside the building.

At the time of the visit, there was no evidence of planned age appropriate activities to keep the children engaged.
The snack table located in the arts and crafts room was not washed and disinfected before the 2-5 group used the table for snack time. All eating surfaces must be washed and disinfected before and after each use.
Children in both groups were observed walking around the classrooms and engaged in activities while eating pre-packed Scooby Graham Crackers. The licensee must ensure that snacks and meals are prepared and served in a safe and sanitary manner.
Outdoors, there were 6 picnic tables observed with peeling paint.
The location of the allergy list was not posted in the classrooms.
There are two metal hooks sticking out of the railing leading into the Ramsey Lodge that are hazards to children. -There is a gas grill with propane attached accessible to children. -The barrier between the lake and the Ramsey lodge has sections that there are no barriers and the children have access to the water which is a hazard.

The Jenkins Lodge did not have a CPR mouthguard in their first aid kit as required.,
The large metal cabinet in the Ramsey Lodge with the chalk board front is broken and tippable.
The Licensee did not have a fire inspection certificate from the local fire department.
A review of five children's records indicated the following: -One did not have documented on an IHCP the potential consequences of not administering the medication to the child. -One did not have the storage directions of the medication. -

Evacuation procedures posted at each exit
In the Ramsey Lodge in the front room the program did not have evacuation procedures posted next to the exit.
The licensee did not have documented on the February 2021 evacuation drill the route taken to exit the building. The educators must document the date, time, exit route used, number of children evacuated and effectiveness of each of the drills.
